Patient Services Statistics. The purpose of this procedure is to provide a complete and accurate record of all off-site patient services/treatment requested by detention staff for which fees for service are billed to Marin County. Entries may be made at time of service rendered, or may be compiled from calendars, Daily Status Report, or appointment book on a periodic basis  Hospital/ER referrals, including medical reason, are documented on the Inmates Sent to Hospital log sheet. Hospital admittances are recorded on the Inmates Sent to Hospital log sheet, and tabulated and referred to the Nurse Manager.  Offsite MD visits are tabulated and reported to Custody every month. Responsibility & Frequency: The Night Shift initiates the 24 hour report by template from the previous 24 hours. Nurses on shift are responsible for updating patient medical information and treatment plans as they change or are discontinued as needed. Reports maintained for 1 month at a time for reference and data retrieval as needed for end-of-month statistical information. DIABETICS Insulin injection times, frequency of BG test, stable, unstable, order diet, snack, needs teaching, etc. TREATMENT/XXXXX XXXXX/LABS /ACTIVITY Items to be performed or monitored during the day/shift dressings, lab specimens to be collected, tests to be done, patients on bed rest, suture removal, cast check, etc. PREGNANT & CHRONIC STABLE (pregnant women on top of list): Methadone maintenance alcohol/drug use, bleeding, infection, etc. INTAKE SCREENINGS List all screenings. If completed - if no medical problems - if any follow-up needed COMMUNICABLE DISEASE Tracking statistical source data for HIV, HEP C, MRSA and TB INFO VERIFICATION Indicate any follow-up necessary to verify unclear orders, new patient meds, continuation of treatment, appointment date, time, etc. SHIFT-to-SHIFT COMMUNICATIONS Space available to make notes to the coming RNs about notable events of the day; any other significant info that needs to be reported.   • Development Services During the term of this Agreement, the Provider agrees to provide to or on behalf of the Port the professional services and related items described in Exhibit A (collectively, the “Development Services”) in accordance with the terms and conditions of this Agreement.
