Notice of Address Change. The Subscriber must notify CareFirst BlueChoice within fifteen (15) days of a change in residence or change in e-mail address, if the Member has consented to receive notices via electronic mail, or as soon as reasonably possible. Except in the case of a covered child who does not reside with the Subscriber, CareFirst BlueChoice is only responsible for mailing notices or correspondence to the last known physical address or e-mail address of the Subscriber.

Any notice sent by the Employer shall be sent by registered mail to the address most recently given by the employee as her home address. Such notice will be deemed to be received by the employee on the second (2nd) postal day following its being sent.
