Discharge processes. If surgery was successful and no problems were identified at the post-operative telephone follow-up, no further specific follow-up is required. The patient will be discharged into the community healthcare system and will receive a copy of two letters sent by the Provider (surgery and follow-up); one directly to their referring optometrist and the other to the patient’s GP detailing the relevant points of the patient’s clinical event. The letter for the referring optometrist will also request the optometrist to measure the patient’s final best corrected distance and near visual acuity and subjective refraction, in order to prescribe glasses, if necessary. For the purposes of collecting clinical outcomes data, the optometrist will be asked to return this measurement to the Provider using the stamped addressed envelop provided. The discharge letter doubles as the patients discharge summary and will contain the following information:  patient demographics  dates of referral or assessment or surgery  date of discharge  diagnosis  what operation was performed  anaesthetic used  IOL (Intra Ocular Lens) inserted with power  whether the surgery was complicated or not and a list of complications if they occurred  whether the patient has endopthalmitis or any other co-morditity  drugs used after surgery  nature (doctor, nurse) of follow up visit and date thereof  pre-op versus post op visual acuity (best corrected if possible)  new allergies  name of patient’s responsible lead healthcare professional at the time of their discharge  medication prescribed at time of discharge  advice on any future management  requirements for second eye surgery  any other relevant or necessary information or instructions  contact details for the patient  any immediate post-discharge requirement from the GP or referrer or other healthcare or social services provider  whether the patient has any relevant infection  the name and position of the person to whom questions about the contents of the discharge letter are addressed and complete and accurate contact details (including telephone number) for that person  whether a certificate of sickness was issued If the original referral covers both eyes, and the second eye is also not a low priority procedure, the patient can be listed for second eye surgery.   • Grievance Processing Union stewards or Union officials shall be permitted to have time off without loss of pay for the investigation and processing of grievances and arbitrations. Requests for such time off shall be made in advance and shall not be unreasonably denied. The Union will furnish the Employer with a list of Union stewards and their jurisdictions.
