Xxxxxx and Vision Insurance. The Board shall contribute to the premium of a group Dental Plan one hundred eighty two dollars ($182.00) per insurance year for each teacher enrolling in the Plan. Teacher enrollment in the Dental Plan shall be voluntary. Any additional premium for the coverage elected by a teacher shall be paid by the teacher through payroll deduction. The Board shall contribute thirty-nine dollars ($39.00) to the premium of the group Vision Plan per insurance year for each teacher enrolling in the plan. Teacher enrollment is voluntary. Any additional premium for the coverage elected by the teacher shall be paid by the teacher.

Xxxxxx and Vision Insurance. The School District will provide dental and vision coverage under a plan offered through the ACSHIC, or any successor plan. The current dental plan includes Riders A, B, C and D. The coverage provided under each of the plans referenced above shall be the plan design established by the ACSHIC (or the entity offering the plans, if applicable), or their successor plans, including as the plan design may be changed from time to time by the ACSHIC. Employees will make a 16 percent contribution of the total monthly premium cost for vision and/or dental coverage, based on the level of coverage elected.

In addition, Landlord may, in its discretion, require Tenant to obtain a lien and completion bond or some alternate form of security satisfactory to Landlord in an amount sufficient to ensure the lien-free completion of such Alterations and naming Landlord as a co-obligee.

  • ’ Compensation Insurance PURCHASER shall perform the operations in accordance with the requirements of the Workers' Compensation Law of the State of Oregon during the term of this contract. In addition, the PURCHASER, its subcontractors, if any, and all employers providing work, labor, or materials under this contract are subject employers under the Oregon Workers' Compensation Law and shall comply with ORS 656.017 and 656.029, which requires them to provide workers' compensation coverage that satisfies Oregon law for all their subject workers. Out-of-state employers must provide Oregon workers' compensation coverage for their workers who work at a single location within Oregon for more than 30 days in a calendar year. Contractors who perform the operations without the assistance or labor of any employee need not obtain such coverage.

  • Workers’ Compensation Insurance Contractor shall obtain and maintain a policy of workers’ compensation insurance for all of Contractor’s employees in accordance with the provisions of Labor Code Sections 3700, et seq., and all other applicable laws and requirements. In case any class of employee is not protected under the workers’ compensation laws for any reason, Contractor shall provide adequate coverage as shall be necessary for the protection of such employees. Prior to commencement of the Work, Contractor shall sign and file with District a certification regarding insurance for workers’ compensation in accordance with Labor Code Section 1861.

Effective the first pay period after 9/1/2011 or as soon as is practicable, for employees hired on or after 9/1/11, employees shall contribute 20% of the illustrated rate for the coverage the employee selects.

  • Liability and Worker’s Compensation Insurance During the Contract term, the Contractor at its sole expense shall provide commercial insurance of such a type and with such terms and limits as may be reasonably associated with the Contract, which, at a minimum, shall be: workers’ compensation and employer’s liability insurance per Florida statutory limits (currently $100,000 per accident, $100,000 per person, and $500,000 policy aggregate) covering all employees engaged in any Contract work; commercial general liability coverage on an occurrence basis in the minimum amount of $500,000 (defense cost shall be in excess of the limit of liability), naming the State as an additional insured; and automobile liability insurance covering all vehicles, owned or otherwise, used in the Contract work, with minimum combined limits of $500,000, including hired and non-owned liability, and $5,000 medical payment. Providing and maintaining adequate insurance coverage is a material obligation of the Contractor and is of the essence of the Contract. The Contract shall not limit the types of insurance Contractor may desire to obtain or be required to obtain by law. The limits of coverage under each policy maintained by the Contractor shall not be interpreted as limiting the Contractor’s liability and obligations under the Contract. All insurance policies shall be through insurers authorized to write policies in Florida.

  • Income Protection Insurance The Employer shall provide Income Protection Insurance through an ETU nominated policy and scheme. It is agreed that the premium will be collected and administered by the “Protect” Severance Scheme at the same time as severance payments are made. Income protection will be paid for the employees and will be paid for all periods of authorised absence and cannot be on a pro-rata basis. It is agreed the Income Protection Insurance payments are paid on a monthly basis by the 14th day of each month. It is agreed that if the Employer has not made a valid or current insurance payment to “Protect”, the Employer shall be liable for any loss of earnings or benefits that would have otherwise been given to the employee.
