Target Population and Enrollment. Grantee will target a coverage-eligible population that includes those attributed to primary care practices in the Health Services Area and individuals identified as uninsured or underinsured at the point of care. Navigators or Community Health Teams may work with health systems when available to query their panel of patients to identify those who are coverage-eligible, including working with the primary care practices, addictions treatment centers, hospitals, or emergency departments.

The target population of Beehive Science & Technology Academy will be represented by various school districts. Currently, our existing school is a school of choice and 63% of students are from Canyons School District, 14% are from Jordan School District and 16% are from Granite School District areas.

Target Population and Enrollment. The Subrecipient’s target population is all residents in Franklin and Grand Isle counties of Vermont plus approximately 1,500 employees of Xxx Peak Resort. The Subrecipient shall successfully assist and/or enroll 2,000 individuals: 0 by 9/30/14; 750 by 12/31/14; 750 by 3/31/15; and 500 by 6/30/15. Subrecipient shall assist members of the target population and the general public to submit at least 2,500 applications and shall conduct 7,500 individual consultations from July 1, 2014 through June 30, 2015.
Target Population and Enrollment. The Subrecipient shall target a coverage-eligible Xxxxxxx County, Vermont population that includes those attributed to primary care practices in the Health Services Area, Small Businesses, sole proprietors, and individuals identified as uninsured and underinsured who are eligible for health insurance enrollment through VHC because of a Qualifying Event (QE) or who need to re-enroll in Medicaid. Support for re-enrollment will be given to those Vermonters currently in VHC who must re-enroll during Open Enrollment between November 15, 2014 and February 15, 2015 for health insurance coverage. It is anticipated that the Subrecipient shall complete 600 enrollments: 50 by 9/30/14; 350 by 12/31/14; 100 by 3/31/15; and 100 by 6/30/15. It is anticipated that there will be 600 applications submitted and 300 individual consultations will be completed from July 1, 2014 through June 30, 2015.
Target Population and Enrollment. The Subrecipient’s target population includes the general public, small businesses, those with Qualifying Events (QE), and other Assistors. The geographic reach for each audience varies. The reach for the general public will include Central and Northeastern Vermont, primarily Washington, Orange, Orleans, and Caledonia Counties. Presentations for small businesses will be focused in Washington County, with enrollment assistance available to all who request it. Outreach will be statewide for those experiencing QEs, primarily due to birth of a child or release from prison.
