Student Success Plans. In conjunction with the work done by the Academic Progress Committee and the redraft of student monitoring and academic progress correspondence, Student Success Plans were developed. The Student Success Plan allows program and academic coordinators, leaders, and/or managers to record tailored intervention strategies for students to support and guide students. Evaluation of these strategies, including follow-up with students who do not continue with their studies to better understand the reasons for this decision Strategies focused on academic orientation and transition in higher education (ie FedReady, PASS, Mentoring), are evaluated using quantitative and qualitative methodology at the end of each teaching semester. This includes evaluation methodology using surveys, focus groups and peer-to-peer feedback. Evaluation and impact are reported at the directorate level and within six-monthly university plan updates: University Learning, Teaching and Student Success Plan (2018-2020) and the Student Retention and Success Operation Plan (2018-2020). Strategies focused on student retention in higher education (ie Data Analytics Dashboard, Student Success Plans and Targeted Call Campaigns) are evaluated at the school and/or central service level. These strategies are evaluated at the local level of implementation and reviewed six-monthly. School led initiatives are evaluated by senior school leaders within learning and teaching committees. Central service led initiatives are evaluated by managers and/or coordinators within the Administration, Retention and Success department, within Student HQ (University Registrar Directorate). School and Student HQ key stakeholders, play a significant role in supporting students who are considering taking leave from studies (See website link). Students are encouraged to discuss their challenges with school and/or central service staff using a variety of communication methods. Students are provided with a range of options to further encourage remaining with their study and the university. Students who take leave of studies or withdraw are logged within a university database and are followed up (ie Outbound Call Campaign) within an appropriate timeframe regarding change of circumstance or willingness to reengage with the university. Evaluation of reasons specific to withdrawals are reviewed within the Data and Reporting Team and made available to senior school leaders as appropriate.

  • Lesson Plans Each teacher shall develop lesson plans for the instruction of students enrolled in his/her classroom. The primary purpose of lesson plans is to assist the classroom teacher with instruction. It also provides the basis to ensure that the state/county curriculum is being presented.

  • Payment Plans Employees covered by the Samaritan Choice medical insurance plan who have outstanding balances that are payable to Samaritan Health Services for in network, covered, and authorized (if medically necessary) services will be provided payment plan offerings upon request from the employee. The request will be made to Patient Financial Services, and may be directed through the Hospital Patient Financial Counselor. Patient Financial Services will work with employees to identify the appropriate payment arrangement based on the employee financial needs/eligibility. Within 120 days from first patient statement, employees must contact Patient Financial Services and identify themselves as a SHS SEIU member and ask for a payment plan arrangement that does not exceed six percent (6%) of their household income. Such requests will be granted using the existing SHS payment options and funding programs. To be eligible for a payment plan, employees must comply with all requirements for establishing appropriate payment options/eligibility, including the completion of a financial assistance application with supporting documentation. Employees who comply with all terms of the payment plan(s) will not be subject to collections or wage garnishment.

  • Educator Plans: Improvement Plan A) An Improvement Plan is for those Educators with PTS whose overall rating is unsatisfactory.

  • School Improvement Plans The School shall develop and implement a School Improvement Plan as required by section 1002.33(9)(n), Florida Statutes and applicable State Board of Education Rules or applicable federal law.
