REQUESTS FOR A LEAVE OF ABSENCE. Except in an emergency, or where otherwise permitted by law, a request for a leave of absence (e.g., statutory leave, general leave, etc.), and any appropriate and/or required supporting documentation, shall be submitted by the employee, to the Employer at least 30 days in advance of such leave. Such requests will be made by the employee using the leave of absence form provided by the Employer. An employee requesting an extension of a leave of absence must make such request in writing at least seven (7) days before the scheduled end of the current leave using the process and forms as stated above. Nothing herein shall prevent the Employer from establishing an automated/electronic submission and approval leave of absence process. A request for a Leave of Absence must be submitted in writing and must be approved in writing by the employee’s Department Head/Director and Employer Human Resources Department before the leave begins.

  • Application for Leave of Absence 25.01 All leaves of absence without pay and any extension thereof must be applied for in writing to the Mayor or his designee, on forms supplied by the Employer, at least ten (10) working days prior to the proposed commencement of the leave except in serious or unusual circumstances, as determined by the Employer. Notification of the approval or denial of their requested leave shall be given to the employee in writing within five (5) working days after the submission of the request. Any denial of a requested leave shall include the reason for the denial.
