Fast Track NHS Continuing Healthcare Services. Providers accredited and awarded this contract will be required to continue providing care to a citizen when the funding stream changes from Social Care to NHS Funded Care. This contract does not apply to providers who already have a contract to provide NHS continuing healthcare under the existing Framework Agreement (Lot 5). In such cases the existing Framework terms, conditions and prices will apply. Fast Track continuing healthcare is NHS funded care where the individual has been referred by an appropriate clinician because that person has a rapidly deteriorating condition and the condition may be entering a terminal phase. The individual may need NHS continuing healthcare funding to enable their needs to be urgently met, e.g. to enable them to go home to die or to provide appropriate end of life support in their own home. The CCG requires the provision of a responsive, high quality patient focused domiciliary care service, including all elements outlined under personal care services, domestic care and health care services, for individuals eligible for fast track NHS continuing health care. The primary aim of the service is to meet their care needs and to enhance the quality of life for the service user. The provider will work closely with family carers as well as healthcare professionals involved in the person‟s care, e.g. district nurse, GP, and any other health commissioned services such as local Hospices. The provider will ensure the service reflects the ethos and standards outlined in the National End of Life Care Strategy with full account being taken of each person‟s preferences set out in their advance Support/Care plan. Care staff are expected to have received specific end of life care training prior to delivering end of life care, and to receive regular refresher training on at least an annual basis.

  • New Financial Services Each Party shall permit a financial service supplier of the other Party to provide any new financial service of a type similar to those services that the Party would permit its own financial service suppliers to provide under its domestic law in like circumstances. A Party may determine the juridical form through which the service may be provided and may require authorisation for the provision of the service. Where such authorisation is required, a decision shall be made within a reasonable time and the authorisation may only be refused for prudential reasons.

  • Core Services The Company agrees to provide those Core Services to the Municipality as set forth in Schedule “A” and further agrees to the process contained in Schedule “A”.

This plan covers services only if they meet all of the following requirements: • Listed as a covered healthcare service in this section. The fact that a provider has prescribed or recommended a service, or that it is the only available treatment for an illness or injury does not mean it is a covered healthcare service under this plan. • Medically necessary, consistent with our medical policies and related guidelines at the time the services are provided. • Not listed in Exclusions Section. • Received while a member is enrolled in the plan. • Consistent with applicable state or federal law. We review medical necessity in accordance with our medical policies and related guidelines. Our medical policies can be found on our website. Our medical policies are written to help administer benefits for the purpose of claims payment. They are made available to you for informational purposes and are subject to change. When a new service or drug becomes available, when possible, we will review it within six (6) months of one of the events described below to determine whether the new service or drug will be covered: • the assignment of an American Medical Association (AMA) Current Procedural Terminology (CPT) code in the annual CPT publication; • final Food and Drug Administration (FDA) approval; • the assignment of processing codes other than CPT codes or approval by governing or regulatory bodies other than the FDA; • submission to us of a claim meeting the criteria above; and • generally, the first date an FDA approved prescription drug is available in pharmacies (for prescription drug coverage only). During the review period, new services and drugs are not covered.
