The undersigned does hereby certify to the governing board of the District that (1) he/she is a representative of the Contractor, (2) he/she is familiar with the facts herein certified, (3) he/she is authorized and qualified to execute this certificate on behalf of Contractor; and (4) that the following is true and correct:

The Consultant shall at all times comply with the provisions of Education Code section 45125.1, et seq. regarding the submission of employee fingerprints to the California Department of Justice and the completion of criminal background investigations of its employees. Verification of compliance with this section and the Fingerprint and Criminal Background Check Certification that may be required with this Contract, shall be provided in writing to the District prior to each individual’s commencement of employment or performing any portion of the services and prior to permitting contact with any student.
