CONNECTION TO VILLAGE SERVICES. A. No users shall be permitted to connect to the sewer and water mains until the Village or Fox Metro (as applicable) has inspected and approved all such mains, the mains have received final regulatory approval from the Illinois Environmental Protection Agency, if required, and all applicable Village or Fox Metro (as applicable) fees have been paid. The Village shall cooperate with the Owner in a timely manner to obtain all approvals from the Illinois Environmental Protection Agency.

CONNECTION TO VILLAGE SERVICES. No users shall be permitted to connect to the sewer and water mains until the Village has inspected and approved all such lines and the lines have received final regulatory approval from the Illinois Environmental Protection Agency and all applicable Village fees are paid. The Village agrees to allow the continued use of all of the existing xxxxx and septic systems serving the Territory until any of the following occur: 1) a well or septic system serving the Territory is no longer serviceable; 2) the Owner/Developer seeks approval of a permit to construct any new building or structure on the Territory or a permit to expand the square footage of an existing structure; or 3) the Agreement is voluntarily or involuntarily terminated. Upon the occurrence of the any of the foregoing events, the Owner/Developer shall, at its sole cost, connect to Village sewer and water facilities in accordance with the Village Code, as amended. The Owner/Developer shall be responsible for paying to the Village its then current tap-on/connection fee pursuant to applicable Village ordinances, as amended. The tap- on/connection fees shall be paid prior to the issuance of any building permit.

  • Ordering of Other UNE Services 2.9.4.1 All LSRs issued for reserved facilities shall reference the facility reservation number as provided by BellSouth. Choice Telephone Company will not be billed any additional LMU charges for the loop ordered on such LSR. If, however, Choice Telephone Company does not reserve facilities upon an initial LMUSI, Choice Telephone Company’s placement of an order for an advanced data service type facility will incur the appropriate billing charges to include service inquiry and reservation per Exhibit B of this Attachment.

  • ELECTRICAL SERVICES A. Landlord shall provide electric power for a combined load of 3.0 xxxxx per square foot of useable area for lighting and for office machines through standard receptacles for the typical office space.

  • Loop A transmission path that extends from a Main Distribution Frame or functionally comparable piece of equipment in a Customer's serving End Office, to the Rate Demarcation Point (or NID if installed at the Rate Demarcation Point) in or at the Customer's premises. The actual transmission facilities used to provide a Loop may utilize any of several technologies.

  • Hospice Services Services are available for a Member whose Attending Physician has determined the Member's illness will result in a remaining life span of six months or less.

  • Outpatient emergency and urgicenter services within the service area The emergency room copay applies to all outpatient emergency visits that do not result in hospital admission within twenty-four (24) hours. The urgicenter copay is the same as the primary care clinic office visit copay.

  • Special Services Should the Trust have occasion to request the Adviser to perform services not herein contemplated or to request the Adviser to arrange for the services of others, the Adviser will act for the Trust on behalf of the Fund upon request to the best of its ability, with compensation for the Adviser's services to be agreed upon with respect to each such occasion as it arises.

  • Surgery Services This plan covers surgery services to treat a disease or injury when: • the operation is not experimental or investigational, or cosmetic in nature; • the operation is being performed at the appropriate place of service; and • the physician is licensed to perform the surgery. Preauthorization may be required for certain surgical services. Reconstructive Surgery for a Functional Deformity or Impairment This plan covers reconstructive surgery and procedures when the services are performed to relieve pain, or to correct or improve bodily function that is impaired as a result of: • a birth defect; • an accidental injury; • a disease; or • a previous covered surgical procedure. Functional indications for surgical correction do not include psychological, psychiatric or emotional reasons. This plan covers the procedures listed below to treat functional impairments. • abdominal wall surgery including panniculectomy (other than an abdominoplasty); • blepharoplasty and ptosis repair; • gastric bypass or gastric banding; • nasal reconstruction and septorhinoplasty; • orthognathic surgery including mandibular and maxillary osteotomy; • reduction mammoplasty; • removal of breast implants; • removal or treatment of proliferative vascular lesions and hemangiomas; • treatment of varicose veins; or • gynecomastia. Preauthorization may be required for these services.

  • Allocation and use of scarce resources Any procedures for the allocation and use of scarce resources, including frequencies, numbers and rights of way, will be carried out in an objective, timely, transparent and non-discriminatory manner. The current state of allocated frequency bands will be made publicly available, but detailed identification of frequencies allocated for specific government uses is not required.
