Check-Off for Life Insurance. The Corporation shall provide a monthly check-off of insurance premiums payable on life insurance plans provided by the Union for its members upon production of appropriate documentation, provided the amounts so deducted are separate from the Union dues. There will therefore be two (2) separate monthly deductions for the Union dues and insurance premiums. The Corporation will not levy a charge upon the Union for rendering this service. The deductions from an employee's pay for a Union insurance program is revocable on notice in writing from the employee to the Corporation or the Union.
