  • Certification of Completion Certification of completion of the program will be contingent upon Resident successfully completing the core requirements of the Program, returning all property of the Medical Center such as books, equipment, etc., and having settled any other professional or financial obligations to the Medical Center. All objectives must have a minimum status of satisfactory progress. • All evaluations must be completed in accordance with ASHP Accreditation Standard as outlined in the Resident Agreement • Completion of pharmacokinetic certification by October 31 of residency year • Completion of inpatient anticoagulation certification by October 31 of residency year. • Completion of major project, which includes meeting all project deadlines and submission of written manuscript of completed project to the RAC. • Assigned projects (MUE, drug review, class review, and management project) have been completed • Presentation of at least one major CME program. • Completion of minimum staffing requirements as described in section 2.7 • Adherence to the organization’s attendance standards and as described in this document under Exhibit A.

  • CERTIFICATION OF INDEPENDENT PRICE DETERMINATION By submission of this bid, the Bidder certifies, and in the case of a joint bid each party thereto certifies as to its own organization, that in connection with this procurement:

  • Certificate of Completion The Interconnection Customer shall provide the EDC with a completed copy of the Interconnection Agreement Certificate of Completion, including evidence of the electrical inspection performed by the local authority having jurisdiction. The evidence of completion of the electrical inspection may be provided on inspection forms used by local inspecting authorities. The Interconnection request shall not be finally approved until the EDC’s representative signs the Interconnection Agreement Certificate of Completion.

  • Completion of Evaluation Cycle 1. The summative evaluation rating shall be based upon a preponderance of the evidence, assessed in a holistic manner, that is aligned to the Ohio Educator Standards. Only evidence gathered during the walkthroughs and formal observations that are conducted for the current school year may be used.

  • Certification of Compliance The Owner may permit the use, prior to sampling and testing, of certain materials or assemblies when accompanied by manufacturer's certificates of compliance stating that such materials or assemblies fully comply with the requirements of the contract. The certificate shall be signed by the manufacturer. Each lot of such materials or assemblies delivered to the work must be accompanied by a certificate of compliance in which the lot is clearly identified. Materials or assemblies used on the basis of certificates of compliance may be sampled and tested at any time and if found not to be in conformity with contract requirements will be subject to rejection whether in place or not. The form and distribution of certificates of compliance shall be as approved by the Owner. When a material or assembly is specified by "brand name or equal" and the Contractor elects to furnish the specified "brand name", the Contractor shall be required to furnish the manufacturer's certificate of compliance for each lot of such material or assembly delivered to the work.
