Application for Leave of Absence. All requests for leave shall be in writing, upon the appropriate form prescribed and provided by the District, with all necessary documentation, such as physician's statement of incapacity or prepared study program, attached. Requests for a leave of absence shall be submitted to the immediate supervisor with reasonable advance notice of the intended leave.
