Application for Leave. Any request for a leave of absence shall be submitted in writing by the employee to the EMPLOYER. The request shall state the reason for the leave of absence being requested and the length of time off the employee desires. Authorization, if granted, for a leave of absence shall be furnished to the employee by the EMPLOYER, and it shall be in writing.
Application for Leave. A request for Family Medical Leave must be made in writing by completing the Family Leave application form. (See appendix J) The application must be submitted to the faculty member’s administrator and then forwarded to the Office of Human Resources at least thirty days before the requested start of the leave unless the reason for the leave is due to an emergency, in which case the request must be made immediately. The completed application must state the reason for the leave and the beginning and ending dates of the leave.

If a need for retraining exists, the appropriate administrator, in consultation with the faculty member, will prepare a proposed retraining plan. The appropriate administrator will submit a recommended plan for consideration to the College President or his/her designee. Retraining plans are subject to the approval of the Chancellor and the Board of Trustees.
