Common use of Application for Leave of Absence Clause in Contracts

Application for Leave of Absence. An employee who requests an unpaid leave of absence shall make written application for such leave on a form provided by the District and shall submit such form to the employee’s Xxxx, Director or Manager as far in advance as possible. The application for leave shall state the leave category requested, the reason(s) necessitating the employee’s absence, and the period of time for which the leave is requested. The disposition of such request will be provided to the employee in writing by an appropriate official of the District. If leave is denied, the employee may submit a request to the appropriate Vice President for disposition.

If leave is denied, the employee Officer may submit a his request to the appropriate Vice President President, Administrative Services for disposition.
