Door 6 – Behavior The repetitive display of behavioral challenges, OR the experience of delusions or hallucinations, both of which are supported by the Preadmission Screen Annual Resident Review (PASARR) requirement for nursing facility admission if the applicant chooses a residential setting for care, that impact the applicant’s ability to live independently in the community and are identified in Door 6.
This section implements sections 1919(b)(3)(F) and 1919(e)(7) of the Act by specifying the terms of the agreement the State must have with the State mental health and Intellectual Disability authorities con- cerning the operation of the State’s preadmission screening and annual resident review (PASARR) program.
The applicant must have exhibited any one of the above behavioral symptoms in at least four of the last seven calendar days (including daily) from the date the LOCD was conducted OR the applicant exhibited delusional thinking or clearly demonstrated having experienced hallucinations within seven calendar days from the date the LOCD was conducted AND met the PASARR requirement for nursing facility admission if they choose a residential setting of care to qualify for Door 6.
PreAdmission Screening/Annual Resident Review (PASARR) - the process by which the State determines whether an individual who resides in or seeks admission to a Medicaid-certified Nursing Facility has, or is suspected of having, mental illness or mental retardation, and, if so, whether the individual requires specialized services.
If the Level I assessment indicates the need for a PASARR Level II assessment of need for specialized services for mental illness and/or mental retardation, the individual must undergo the PASARR Level II assessment.
All individuals who reside in or seek admission to a Medicaid-certified Nursing Facility must have a PASARR Level I assessment for mental illness and mental retardation.
Preadmission screening and an- nual resident review (PASARR) activi- ties conducted by the State: 75 percent.
Door 6 Eligibility Requirement: The applicant must have exhibited any one of the above behavioral symptoms in at least four of the last seven (7) calendar days (including daily) from the date the LOCD was conducted online OR the applicant exhibited delusional thinking or clearly demonstrated having experienced hallucinations within seven (7) calendar days from the date the LOCD was conducted online AND met the PASARR requirement for nursing facility admission if they choose a residential setting of care.
It also means the date of the determination made by a State with regard to the PASARR requirements of Section 1919(e)(7) of the Social Security Act.
Direct transfer from a hospital—If a physician attests that the individual is likely to need thirty (30) days or less of nursing facility care for the condition for which the individual was hospitalized, no Level II screening is necessary and the individual is exempt from the PASARR process.
