Waiting Period/Each Occurrence. Accrued ESDP shall be used 7 starting with the 25th consecutive scheduled hour missed, or the 8 fourth scheduled workday missed regardless of hours, whichever 9 comes first, due to personal illness when the illness is sufficiently 10 severe to require the nurse to miss work. However, first hour 11 immediate ESDP access will be provided if the RN is hospitalized 12 either in-patient or for short-stay surgery when surgery time and the 13 recovery period equals at least twenty-four (24) scheduled hours, 14 and in connection with injuries or illness covered by workers’ 15 compensation, as described in Article 11.3(D) hereof. In addition, 16 first hour ESDP access will be granted for an absence due to the 17 recurrence, within a seven (7) day period of return to work, of the 18 same injury/illness for which the twenty-four (24) hour waiting period 19 has already been satisfied. There will be only one (1) twenty-four 20 (24) hour waiting period each rolling twelve (12) month period for the 21 same medical condition before an RN on intermittent FMLA leave 22 can access ESDP benefits on a first hour basis. 23

  • CONTRACT RENEWAL PERIOD FURNITURE ADDITIONS/DELETIONS: Contractors wishing to make furniture additions/changes to their contract during the contract period should be aware of the following additions/changes will be allowed only once during a contract period. No changes to discount percentages are allowed during this contract period. Additions/changes must be submitted during the contract renewal period. When requesting additions and/or changes; contractor shall submit a request either by letter or via email which shall include a list of all items being proposed for addition and/or change. Contractor shall submit all required documents supporting any new items. Product literature for all new items shall also be submitted with the request. Contractor is responsible for submitting a complete name, address, email address, phone and fax numbers when a new dealer is added.
