SEMI-PRIVATE HOSPITAL. The Employer agrees to provide for Semi-Private hospital care, and O.H.I.P. Supplemental or Supplementary Health Care benefits for active full-time employees on the following basis:
SEMI-PRIVATE HOSPITAL. (F/T) The Employer agrees to pay one hundred percent (100%) of the premium for semi-private hospital coverage.
SEMI-PRIVATE HOSPITAL. The semi-private coverage shall be amended to provide for one hundred (100%) percent reimbursement.
SEMI-PRIVATE HOSPITAL. One hundred per cent (100%) of the premium costs for each employee who participates in the Semi-Private Hospital coverage.
SEMI-PRIVATE HOSPITAL. (a) The premiums for regular full-time, regular part-time and all temporary employees, whether full-time or part-time, are 100% paid by the employee if he/she elects this benefit coverage.
SEMI-PRIVATE HOSPITAL. The Employer agrees to provide for hospital care, and Supplemental or Supplementary Health Care benefits for active full-time employees on the following basis: twenty-five dollars ($25.00) deductible for single, and fifty dollars ($50.00) deductible for family each calendar year. the Plan provides for reimbursement of eighty percent (80%) of Semi- Private hospital costs. the Plan provides for reimbursement of one hundred percent of Prescription Drugs with no deductible for this coverage.
SEMI-PRIVATE HOSPITAL. 100% per day for a maximum of 90 days per calendar year, $175 per day. Prescription Drugs: 90% up to $1,000 per person per calendar year. Vision: $150.00 every 24 months (Township to reimburse). Employees interested in the above coverage must notify the employer in writing of their intent to enrol sixty (60) days prior to their effective retirement date. Employees must be in the plan for six (6) months.
SEMI-PRIVATE HOSPITAL. The Employer agrees to provide for Semi-private hospital care for active full-time employees on the following basis:
SEMI-PRIVATE HOSPITAL. Semi Private hospital room charges covering the differential between the standard xxxx and the semi private room rates.
SEMI-PRIVATE HOSPITAL. The Employer shall make available a semi-private hospital room insurance plan. The premium shall be paid 90% by the employer.
