Project Staff Experience. 280 points available: Submit a completed Individual Resume Form (RFQ, Attachment 2) for each of the following positions below Do NOT submit more than (1) person for each Role and label them specifically as it is labeled below as the “Role in this Project” and label them the same on the Resume Form so they match the Roles outlined below. Do not submit any additional Roles past those as define in this RFP. One (1) individual can perform (2) roles but must be labeled as such on the Resume Form, for instance “Project Manager/Senior Geologist”: An additional single page may also be included behind each form for each resume. The resume must present the individual’s overall experience, education, licensing and other general information as well as the individual’s higher education, and consulting experience, primarily on Community College District projects, and any other public works projects. Use the Resume Form to explicitly define how the individual meets these criteria. The Form must be filled out with the information requested and not dependent on any supplemental resumes or information. All proposed “Senior-Level” personnel must be licensed professionals registered in the State of California, including both the Principal in Charge and Project Manager. The submitting firm shall only propose staff possessing professional licenses and/or certifications that correspond with their assigned role title listed within the following table: The submitting firm shall have at least one individual with a Special Testing certification from any of the following organizations: • Building Special Testing Association (CCP designation) • Provide ICBO Certified Special Inspectors (or AWS/SNT Certified Welding Inspectors) • Field Personnel and Lab Technicians (certified ACI Level I) • Special Inspectors (DSA SSS form 103-1) • Registered Engineers with the State of California The specific individuals submitted with the SOQ may be requested by the District for a future project. # Role in this Project Available Points

Related to Project Staff Experience

  • Project Staff Contractor shall appoint to the Project Staff: (i) individuals with suitable training and skills to provide the Work, and (ii) sufficient staffing to adequately provide the Work. Contractor shall make commercially reasonable efforts consistent with sound business practices to honor the specific request of the JBE with regard to assignment of its employees. The JBE may require Contractor to remove any personnel from the Project Staff that interact with any personnel of the Judicial Branch Entities or JBE Contractors (including, without limitation, the Contractor Project Manager) upon providing to Contractor a reason (permitted by law) for such removal. The Polk County School Staffing Plan shall be constructed each year according to the procedures set forth in Board Policy and, upon adoption, shall become Board Policy.

  • Staffing Consultant will designate in writing to Authority its representative, and the manner in which it will provide staff support for the project, which must be approved by Authority. Consultant must notify Authority’s Contract Representative of any change in personnel assigned to perform work under this Contract, and the Authority’s Contract Representative has the right to reject the person or persons assigned to fill the position or positions. The Authority’s Contract Representative shall also have the right to require the removal of the Consultant’s previously assigned personnel, including Consultant’s representative, provided sufficient cause for such removal exists. The criteria for requesting removal of an individual will be based on, but not limited to, the following: technical incompetence, inability to meet the position’s qualifications, failure to perform, poor attendance, ethics violation, unsafe work habits, or damage to Authority or other property. Upon notice for removal, Consultant shall replace such personnel with personnel substantially equal in ability and qualifications for the positions and shall submit the proposed replacement personnel qualification and abilities to the Authority, in writing, for approval.

  • PROJECT SERVICES Landlord shall furnish services as follows:

  • Physician's Statement Employees requesting Temporary Disability Leave shall submit to the Chief, through the chain of command, a written statement from their physician verifying their condition, recommending limited duty or leave of absence, and describing their limitations and prognosis.
