Processing of Claims, Appeals. Security Benefit will develop and distribute forms and establish procedures for submission of reimbursement claims by eligible participants to Security Benefit under the Program. All claims must be accompanied by proof deemed adequate by Security Benefit under the Cafeteria Plan Requirements to substantiate the amount, nature, and other necessary details of the expense for which the Participant claims reimbursement. If adequate proof of a claim is not provided, or the claim does not meet established guidelines for reimbursable claims, Security Benefit may, at its option, hold the claim pending receipt of adequate or acceptable proof, notify the Participant of a claim denial, or return the claim to the Participant for resubmission with adequate and acceptable proof. Security Benefit will establish further formal claim review and appeal procedures; provided, however, that the Employer will have the right to review such procedures and will remain responsible for adjudication of any appeal of an adverse benefit determination made with respect to an initial claim, in accordance with Section 3.4 of this Agreement. The Employer will also remain solely responsible for determining the individuals who are eligible to participate in the Plan and providing accurate information to Security Benefit regarding eligible Plan participants and dependents. In the event that an individual disputes the Employer’s determination regarding his eligibility to participate in the Plan, the Employer will be responsible for adjudicating any related claim and/or appeal.

  • Claims Submission We will submit your claims and assist you in any way we reasonably can to help get your claims paid. Your insurance company may need you to supply certain information directly. It is your responsibility to comply with their request. Please be aware that the balance of your claim is your responsibility whether or not your insurance company pays your claim. Your insurance benefit is a contract between you and your insurance company; we are not party to that contract.

  • Claims Administration An employee will be required to comply with any and all rules and regulations and/or limitations established by the carrier or applicable third party administrator and contained in the policy, and employees and their dependents shall look solely to such carrier or third party administration for the adjudication of the payment of any and all benefits claims.

  • Claims Administrator A. The Human Resources Director through his/her designated Claims Administrators shall administer the provision of this policy. The City Physician shall provide the City's Claims Administrators with all available medical information concerning the Employee's injury and/or medical opinions as requested. Medical information and opinions shall be based upon the Employee's medical records and/or physical examination. Questions of Employee eligibility shall be determined by the provisions established under State Statute 49-110, 49-111 and Oklahoma Worker's Compensation Title 85. Prior to any denial of injury leave benefits where lost time actually occurred, the administrator shall notify Union and allow a Union representative the opportunity to review the application pending denial and provide any additional information relating to same as may be necessary. Should the City change designated Claims Administrators Local 176 will be notified in writing.

  • Settlement of Claims The Company’s obligation to make the payments provided for in this Agreement and otherwise to perform its obligations hereunder shall not be affected by any circumstances, including, without limitation, any set-off, counterclaim, recoupment, defense or other right which the Company may have against the Executive or others.

Appendix E Appendix E, Report of Contract Purchases, attached hereto, is hereby expressly made a part of this Contract as fully as if set forth at length herein. OGS reserves the right to make unilateral changes to this Report of Contract Purchases document. Appendix F Appendix F, Project Based Information Technology Consulting Services Processes and Forms, attached hereto, is hereby expressly made a part of this Contract as fully as if set forth at length herein. OGS reserves the right to change the processes and forms set forth Appendix F in non-material and substantive ways without seeking a contract amendment. Appendix F is comprised of the following attachments:

  • Appeal Process PROVIDER may appeal any adverse finding by the Contract Compliance Officer as set forth in sec. 25.08(20)(c), D.C. Ords.

  • Claims Review The IRO shall perform the Claims Review annually to cover each of the five Reporting Periods. The IRO shall perform all components of each Claims Review.
