NOTICE OF LEAVE AND EVIDENCE. SUPPORTING A CLAIM The payment of paid personal/carer’s leave is subject to the Employee promptly advising the Employer of the Employee’s absence and its expected duration and in the case of carer’s leave, the name of the person requiring care or support and their relationship to the Employee. Unless a valid reason exists otherwise the Employee should contact the Employer to notify of the absence within four (4) hours of the scheduled start time. This will be required for each rostered shift the Employee is absent. The inability to attend work must be notified to the Employee’s supervisor by telephone call. Text messages, emails etc. are not an acceptable means of notifying the Employer of an absence. Where a supervisor is unavailable it is the Employee’s responsibility to advise alternative senior management by telephone. When an Employee is absent due to sickness the Employee is required to provide documentary evidence for the period of personal/carer’s leave. Documentary evidence will usually be in the form of a medical certificate from a registered health practitioner, if this is not possible a statutory declaration can be provided. The Employer may also request an additional medical opinion as soon as reasonably possible if the Employee did not get the medical certificate from a registered health practitioner of the Employer’s choice. For the purposes of this clause, personal/carer’s leave will be provided and recorded on an hourly basis. Where the prescribed paid personal/xxxxx’s leave is nominated for an Employee each day provided will be considered as 7.6 hours. Each hour that an Employee notifies the Employer they are unavailable to work will be recorded and the personal/carer’s leave hourly balance adjusted accordingly. Where an Employee is on personal/carer’s leave, or not able to work for some reason they are required to keep in contact on a regular and frequent basis ether by email of telephone..

  • Notice of Leave In any case in which the necessity for FMLA leave is based upon an expected birth or placement for adoption, or planned medical treatment for a serious health condition of the employee, family member, or member of the employee’s household, the employee will provide the Superintendent with at least sixty (60) calendar days advance written notice of the date the leave is to begin. When the need for FMLA leave is due to unforeseen circumstances and advance notice is not practicable, the employee will provide notice of the need for such leave as soon as practicable.

  • Notice Requirements and Commencement of Leave (1) An employee who requests parental leave for the adoption or caring of a child shall be required to provide proof of adoption or birth of the child.

  • Verification of Leave Upon return from leave the employee will provide the necessary claim form for verification of absence to his/her principal/supervisor within five (5) workdays in accordance with §1012.61, Florida Statutes.

  • Notice of Legal Actions The Contractor must notify the Department of any legal actions filed against it for a violation of any laws, rules, codes, ordinances or licensing requirements within 30 days of the action being filed. The Contractor must notify the Department of any legal actions filed against it for a breach of a contract of similar size and scope to this Contract within 30 days of the action being filed. Failure to notify the Department of a legal action within 30 days of the action will be grounds for termination for cause of the Contract.

  • Terms of Leave This leave shall be granted, in the following instances, for no more than a total of twelve (12) work weeks during the school year, July 1 through June 30. It shall be granted to eligible bargaining unit members for: (A) to care for the employee's child after birth, or following placement for adoption or xxxxxx care; (B) to care for the employee's spouse, son, daughter, or parent who has a serious health condition; or (C) for a serious health condition that makes the employee unable to perform the employee's job.

  • Conditions of Leave The Town Board will specify the duration of an unpaid leave of absence and to impose such other terms, conditions and restrictions on the employee as the Town Board, in its discretion, deems appropriate.

  • Waiver of Conditions Precedent The conditions specified in this clause 9 are inserted solely for the benefit of the Bank and may be waived by the Bank in whole or in part and with or without conditions.
