Care Management Data Submission Clause Samples
The Care Management Data Submission clause requires parties to provide specific data related to care management activities. Typically, this involves regularly submitting reports or datasets detailing patient care, outcomes, or service utilization to a designated party or system, often in a prescribed format and within set timeframes. Care Management Data Submission. The ICDS plan must submit an electronic file of Care Management data as specified by ODM. In addition the ICDS Plan must submit a description of the Care Management model, Assessment tools, staffing model, care plan templates, etc., for review and approval as specified by ODM. Documentation submitted by the ICDS Plan as well as Care Management records are subject to a review and audit by ODM and/or the external quality review organization (EQRO).
Care Management Data Submission. The MCP must submit a monthly electronic report to the Care Management System (CAMS) for all members who are provided care management services by the MCP as outlined in the ODJFS Case Management File and Submission ▇▇▇▇▇▇▇▇▇▇▇▇▇▇.▇▇ order for a member to be submitted as care managed in CAMS, the MCP must (1) complete the identification process, a comprehensive health assessment and development of a care treatment plan for the member; and (2) document the member’s written or verbal confirmation of his/her care management status in the care management record. ODJFS, or its designated entity, the external quality review vendor, will validate on an annual basis the accuracy of the information contained in CAMS with the member’s care management record. The CAMS files are due the 15th calendar day of each month. The MCP must also have an ODJFS-approved care management program which includes the items in Section 3.b.. Each MCP should implement an evaluation process to review, revise and/or update the care management program. The MCP must annually submit its care management program for review and approval by ODJFS. Any subsequent changes to an approved care management program description must be submitted to ODJFS in writing for review and approval prior to implementation.
