Whole-Person Care – Wellness Center Service DeliveryIn June, 2017, the California Department of Health Care Services (DHCS) announced that the Small-County Whole Person Care (WPC) Collaborative (consisting of the counties of Plumas, Mariposa and San Benito) had been approved and selected to receive second round total funds in the amount of $10,362,176 to implement its local WPC Pilot program.
Participating entities in the WPC Collaborative will reach out to the Drop-In Center providers and organizations providing services to develop plans to further support their efforts.
The San Benito County WPC Collaborative will be composed of the Human Services Department and selected Participating Entities (described further below).
Over the term of the grant, the WPC Collaborative will continue to evaluate these systems as the other innovations are put into place to ensure appropriate bi-directional sharing of information is available to all participating entities including CalOptima.
For Program Year 2, Lestonnac, with 2-1-1 Orange County and the WPC Collaborative, will develop the policies relating to participation and use as well as develop an outreach plan to add additional nonprofit providers to the referral network (focusing on those providing services to homeless persons as a first priority).

For the WPC, they will continue these efforts targeting homeless beneficiaries and providing the knowledge learned through their pilot to the WPC Collaborative, including hospitals newly implementing this model under the WPC.
