Clinical Records definition

Clinical Records means a written account of all services provided a patient by the home health agency, as well as other pertinent information necessary to provide care.
Clinical Records means information concerning or related to the examination or treatment of a person receiving services from a provider agency.
Clinical Records means records created by a licensee regarding the observation and treatment of patients, such as progress notes, but does not include psychotherapy notes.

Clinical Records means any record, hard copy or patient abstract, containing information which could lead to the identification of head and spinal cord injury patients, head and spinal cord injury diagnosis or treatment facilities, independent clinical laboratories, or health care providers.
