Procedure for registering a kinship care agreement. A parent or guardian who has entered into a kinship care agreement may register that agreement with the clerk of the children’s court. A kinship care agreement which is to be registered must be recorded on Form 12A, which is appended to the Child Care and Protection Regulations. The following documents must be submitted to the clerk of the children’s court along with the kinship care agreement: � the child’s birth certificatean affidavit by the parent or guardian confirming that he or she is the child’s legal custodian (with copies of any relevant court order or document) � proof of guardianship status, IF the agreement has been entered into by a legal guardian who is not a parent. An affidavit is a written statement which a person swears to before a commissioner of oaths such as a lawyer or a police officer. The clerk of the children’s court will stamp or endorse the agreement to show that it has been registered and give a copy of the endorsed agreement to each of the parties and to the Ministry. A kinship care agreement must be registered at the children’s court if the kinship care-giver wants to apply for or receive a State maintenance grant for the child. The registration process is intended to make sure that a grant paid for a particular child goes to the person who is actually caring for that child. The clerk of the children’s court must provide a copy of the agreement to the Ministry for this purpose.   • Transition of Registry upon Termination of Agreement text for intergovernmental organizations or governmental entities or other special circumstances: “Transition of Registry upon Termination of Agreement. Upon expiration of the Term pursuant to Section 4.1 or Section 4.2 or any termination of this Agreement pursuant to Section 4.3 or Section 4.4, in connection with ICANN’s designation of a successor registry operator for the TLD, Registry Operator and ICANN agree to consult each other and work cooperatively to facilitate and implement the transition of the TLD in accordance with this Section 4.5. After consultation with Registry Operator, ICANN shall determine whether or not to transition operation of the TLD to a successor registry operator in its sole discretion and in conformance with the Registry Transition Process.   • Pricing for Registry Services (a) With respect to initial domain name registrations, Registry Operator shall provide ICANN and each ICANN accredited registrar that has executed the registry-­‐registrar agreement for the TLD advance written notice of any price increase (including as a result of the elimination of any refunds, rebates, discounts, product tying or other programs which had the effect of reducing the price charged to registrars, unless such refunds, rebates, discounts, product tying or other programs are of a limited duration that is clearly and conspicuously disclosed to the registrar when offered) of no less than thirty (30) calendar days. Registry Operator shall offer registrars the option to obtain initial domain name registrations for periods of one (1) to ten (10) years at the discretion of the registrar, but no greater than ten (10) years.

Upon conclusion of Registry Operator’s designation as operator of the registry for the TLD, all such names withheld from registration or allocated to Registry Operator shall be transferred as specified by ICANN. Registry Operator may self-­‐allocate and renew such names without use of an ICANN accredited registrar, which will not be considered Transactions for purposes of Section 6.1 of the Agreement.

  • Department of State Registration Consistent with Title XXXVI, F.S., the Contractor and any subcontractors that assert status, other than a sole proprietor, must provide the Department with conclusive evidence of a certificate of status, not subject to qualification, if a Florida business entity, or of a certificate of authorization if a foreign business entity.

  • Administration of Agreement All approvals referenced in this Agreement must be obtained from the parties' contract administrators or their designees. All notices must be given to the parties' contract administrators respectively. The OAG's contract administrator is Xxxxxxxxx X. Xxxxxx, Chief, Bureau of Advocacy and Grants Management. The Provider’s contract administrator will be provided at the time of execution. The parties will provide each other with written notification of any change in its designated representative for this Agreement. Such changes do not require a formal written amendment to this Agreement.

  • Reporting of Unauthorized Disclosure The Contractor shall immediately report to the State any unauthorized disclosure of confidential information.
