CONSULTANT ELIGIBILITY. This Agreement solicits Consultants (individuals and/or agencies) who are experts in the areas of Behavioral Health, Alzheimer’s Disease and Related Dementia, Intellectual and Developmental Disabilities, Traumatic Brain Injury, Chronic Mental Illness and/or Substance Use Disorder and has successfully worked with clients who exhibit maladaptive behavior, often dangerous, possibly life-threatening, and may be complicated by medical or mental health factors. In addition to the provisions of Appendices A-E to this Provider Agreement, the Consultant agrees to comply with the provisions of all other applicable state and federal laws; is expected to follow licensing/certification rules for the State of Alaska for their field of practice; agrees to keep current any and all licenses, certifications, and credentials required to qualify for providing services through this Agreement and to keep current the necessary documentation on file with DHSS to demonstrate compliance; declares and represents that they meet the qualifications required for a Consultant for this Agreement; agree to sign additional documentation that relates to the protection and sharing of client information. The Consultant must submit the following documentation along with the signed agreement:

CONSULTANT ELIGIBILITY. This Agreement solicits Consultants (agencies and/or individuals) who are experts in the areas of Behavioral Health, Alzheimer’s Disease and Related Dementia, Intellectual and Developmental Disabilities, Traumatic Brain Injury, Chronic Mental Illness and/or Substance Use Disorder and has successfully worked with clients who exhibit maladaptive behavior, often dangerous, possibly life- threatening, and may be complicated by medical or mental health factors. The CBC program is a multidisciplinary team of professionals with specific skill sets, backgrounds, and areas of expertise that can deliver services to a wide range of clients. Previously approved CBC providers will not automatically be re-approved. Approval will be based on provider specialty, geographic location, and past performance. In addition to the provisions of Appendices A-G of this Provider Agreement, the Consultant agrees to comply with the provisions of all other applicable state and federal laws; is expected to follow licensing/certification rules for the State of Alaska for their field of practice; agrees to keep current any and all licenses, certifications, and credentials required to qualify for providing services through this Agreement and to keep current the necessary documentation on file with DOH to demonstrate compliance; declares and represents that they meet the qualifications required for a Consultant for this Agreement; agrees to sign additional documentation that relates to the protection and sharing of client information; and has a signed Memorandum of Agreement (MOA) between the Consultant and the Service Provider or Project receiving services through the Agreement (Attachment 3).   • Service Eligibility A bonus authorized by subsection (a) may be paid to a person or offi- cer only if the person or officer agrees under subsection (d)—

  • Client Eligibility Client eligibility and service referral are the responsibility of DDA under chapter 388- 823 WAC (Eligibility) and chapter 388-825 WAC (Service Rules). Only persons referred by DDA shall be eligible for direct Client services under this Program Agreement. It is DDA’s responsibility to determine and authorize the appropriate direct service(s) type. Direct Client services provided without authorization are not reimbursable under this Program Agreement.

  • Dependent Eligibility To be eligible to enroll as a Covered Dependent, a person must be:

  • Overtime Eligibility An Employee must work at least fifteen (15) minutes beyond her normal shift before being eligible for overtime compensation.

  • Benefit Eligibility For purposes of the Benefit Plan entitlement, common-law and same sex relationships will apply as defined.

  • Employee Eligibility For purposes of this section, “eligible employee” shall be defined by the Public Employees’ Medical and Hospital Care Act.

  • Verification of Employment Eligibility By executing this Agreement, Consultant verifies that it fully complies with all requirements and restrictions of state and federal law respecting the employment of undocumented aliens, including, but not limited to, the Immigration Reform and Control Act of 1986, as may be amended from time to time, and shall require all subconsultants and sub-subconsultants to comply with the same.

  • Student Eligibility A. The Texas Success Initiative (TSI) requires mandatory assessment for all students to determine college readiness in reading, writing and math. The xxxx authorizes the Texas Higher Education Coordinating Board to prescribe assessment instruments with a statewide passing standard. The initiative allows an institution to determine when a student is ready to perform college‐level coursework. High School students who seek to register in a dual credit course, which will grant college credit must prove “college readiness” by achieving a college level score as outlined in Appendix A.

  • Employees - Basic Eligibility Employees may participate in the Group Insurance Program if they are scheduled to work at least 1044 hours in any twelve consecutive months, except for:

  • Member Eligibility Verify Member eligibility contemporaneous with the rendering of services. BCBS will provide systems and/or methods for verification of eligibility and benefit coverage for Members. This is furnished as a service and not as a guarantee of payment;
