Patient Personal Data definition

Patient Personal Data means any Personal Data and/or Special Category Personal Data of any Patient(s) that fall(s) within any of the categories of Personal Data specified in paragraph Error! Reference source not found. (Categories of Personal Data - Patients) of Schedule 1 (Data Sharing Protocol)) and means in particular such data as any Disclosing Party makes available under this Agreement and that a Receiving Party receives under this Agreement;
Patient Personal Data means any Personal Data pertaining to EU Individuals who have Zimmer Biomet devices, or those considering using such devices.
Patient Personal Data means any Personal Data of any Patient(s) that fall(s) within any of the categories of Personal Data specified in paragraph 5.1 (

Examples of Patient Personal Data in a sentence

  • Without prejudice to the generality of clause 7.1, you will ensure that you have all necessary appropriate consents and notices in place to enable lawful transfer of the Patient Personal Data to us and/or lawful collection of the Patient Personal Data by us or on your behalf for the duration of and for the purposes of this Contract.

  • We will not be liable to you whether in contract, tort (including negligence), breach of statutory duty, or otherwise under or in connection with the Contract for any liability, loss, damage, costs or expenses incurred by you as a result of inaccurate Patient Personal Data being extracted from your systems due to an error or inaccuracy in the SNOMED CT clinical codes on your system from which we extract the Patient Personal Data.

  • SCHEDULE 1 - PERMITTED PURPOSES AND AUTHORISED USERS Permitted Purposes Permitted Purposes in relation to Patient Personal Data [The provision, directly to the relevant Patient, of healthcare by any Registered Healthcare Professional who is a member of Staff of any Party.

Related to Patient Personal Data

  • Client Personal Data means any personal data provided to us by you, or on your behalf, for the purpose of providing our services to you, pursuant to our engagement letter with you;

  • Customer Personal Data means the personal data contained within the Customer Data.

  • Personal Data means any information relating to an identified or identifiable natural person (‘data subject’); an identifiable natural person is one who can be identified, directly or indirectly, in particular by reference to an identifier such as a name, an identification number, location data, an online identifier or to one or more factors specific to the physical, physiological, genetic, mental, economic, cultural or social identity of that natural person;

  • Shared Personal Data means the personal data to be shared between the parties under clause 1.2 of this agreement to enable the parties to fulfil their obligations under the terms of this Agreement.

  • Personal Data Breach means a breach of security leading to the accidental or unlawful destruction, loss, alteration, unauthorised disclosure of, or access to, personal data transmitted, stored or otherwise processed;

  • Student Personal Information means information collected through a school service that personally identifies an individual student or other information collected and maintained about an individual student that is linked to information that identifies an individual student, as identified by Washington Compact Provision 28A.604.010. For purposes of this DPA, Student Personal Information is referred to as Student Data.

  • Patient Data means any electronic data, information or material about a Patient entered into the Software.

  • Personal Information means information identifiable to any person, including, but not limited to, information that relates to a person’s name, health, finances, education, business, use or receipt of governmental services or other activities, addresses, telephone numbers, Social Security Numbers, driver license numbers, other identifying numbers, and any financial identifiers.
