Part 2 Information means any records containing information, whether recorded or not, received or acquired by a Part 2 Program that identifies an Individual as a recipient of services from a Part 2 Program. (e.g., diagnosis, Treatment and referral for Treatment information, billing information, emails, voice mails, and texts). Essentially, Part 2 Information will state or suggest the Individual has a Substance Use Disorder (SUD) or has been treated by a Part 2 Program.
Part 2 Information means information protected by 42 C.F.R. Part 2, as more specifically set forth at 42 C.F.R. §§ 2.11 and 2.12(a).
Part 2 Information means any information subject to the regulations under 42 CFR Part 2.

Contractor shall not redisclose Part 2 Information to a third party unless the third party is a contract agent of Contractor helping Contractor provide services under the Service Contract and only as long as the agent further discloses Part 2 Information only back to Contractor or SBHASO.
Contractor acknowledges that any unauthorized disclosure of Part 2 Information may be a federal criminal offense.
PHI includes, without limitation, Electronic PHI, mental health information, sexually transmitted disease information, and Part 2 Information.

Part 2 Information means alcohol abuse, drug abuse, or substance use disorder information covered by Part 2. “PHI” shall have the same meaning as the term “Protected Health Information” that is created, received, maintained, or transmitted by Contractor from or on behalf of SBHASO. PHI includes, without limitation, Electronic PHI, mental health information, sexually transmitted disease information, and Part 2 Information. “PII” means personally identifiable information as defined under Washington law.
