General Conditions of Coverage. (Short Term) – In order to have the sick leave period reinstated up to 130 days, an employee who has been on sick leave absence must return to work for one (1) full day in the case of an unrelated and different disability cause and for thirty (30) calendar days in the case of the same disability cause. – Medical certificate(s) as per Appendix "A" attached hereto may be required as requested by the Employer. Forms will be available from Supervisors, Payroll or Human Resources. The Employer shall be responsible for payment of completed medical certificate, if required. – Successive absences due to the same or related cause may be considered as one continuous period of disability if the employee has met the qualifying conditions for LTD coverage.

  • Conditions of Use means Conditions 1 to 10 and the Schedules and Appendices of this document.

  • Levels of cover means cover is available for either hospital treatment, general treatment or combined hospital and general treatment.

  • Conditions of Sale means the Conditions of Sale set out in Part 3 of this TenderDocument;

  • Special Conditions means Special Conditions of Contract, which override the General Conditions, also referred to as SCC.

  • Terms & Conditions means the Special Conditions of Contract and General Conditions of Contract herein mentioned and other stipulations incorporated in any part of Tender Document and /or Agreement.

  • Conditions of Service means the document developed by a distributor in accordance with subsection 2.4 of the Code that describes the operating practices and connection rules for the distributor; (DSC)

  • Conditions of Carriage means these conditions of carriage.

  • Specific Conditions means the conditions in addition or in variation to the general conditions which the Commission may lay down specifically for a distribution licensee;

  • Supplemental Conditions means those terms and conditions, if included in the Agreement by mutual written agreement of the Parties, which add to or modify the Agreement and are incorporated by reference as if fully set forth in the Agreement. In the case of a conflict between the Supplemental Conditions and the Agreement, the Supplemental Conditions shall prevail.

  • Additional Conditions means the terms and conditions set out in the Offer Document.

  • Conditions Precedent means the conditions precedent set out in clause 3 below;

  • Period of Coverage means the Plan Year, with the following exceptions: (a) for Employees who first become eligible to participate, it shall mean the portion of the Plan Year following the date on which participation commences, as described in Section 3.1; and (b) for Employees who terminate participation, it shall mean the portion of the Plan Year prior to the date on which participation terminates, as described in Section 3.2.

  • Conditions of Tender means the Conditions of Tender for the said land as set out in Annexure A;

  • Evidence of coverage means any certificate, agreement or contract issued to an enrollee setting out the coverage to which the enrollee is entitled.

  • Acquisition and Construction Fund means the fund so designated in, and created pursuant to, Section 502 hereof.
