Xxxxxx Youth/NMD Records. 15 9.1.1 CONTRACTOR shall prepare and maintain accurate and complete records 16 on each Xxxxxx Youth/NMD served under the terms of this Agreement in a form acceptable to 17 ADMINISTRATOR. In addition to the records required to be maintained by Title 22 CCR, 18 CONTRACTOR shall also maintain the following information in Xxxxxx Youth’s/NMD’s case 19 files: 20 Information regarding the Xxxxxx Youth’s/NMD’s participation 21 in TPSP, as applicable; 22 Statement of behaviors with potential risk and/or safety 23 concerns; 24 Xxxxxx Youth’s/NMD’s financial information, including 25 disbursements for clothing and material signed for by Xxxxxx Youth/NMD, and allowances 26 received by and signed for by Xxxxxx Youth/NMD; 27 Diagnostic studies; 28 Reports on interviews with Xxxxxx Youth/NMD; 4 professionals; 5 Special Incident Report; Written quarterly evaluations; Written verification of clinical services provided by treatment Records of medical/dental visits and treatment, including a copy 6 of CHDP physical or its equivalent that is less than one (1) year old;

Xxxxxx Youth/NMD Records. 11.1.1 CONTRACTOR shall prepare and maintain accurate and complete records on each Xxxxxx Youth/NMD served under the terms of this Agreement in a form acceptable to ADMINISTRATOR. In addition to the records required to be maintained by Title 22 CCR, CONTRACTOR shall also maintain the following information in Xxxxxx Youth’s/NMD’s case files: Information regarding the Xxxxxx Youth’s/NMD’s participation in TPSP/ILP, as applicable; Statement of behaviors with potential risk and/or safety concerns; IPC referral for STRTP placement and/or admission statement; Initial and monthly CFT reports; Xxxxxx Youth’s/NMD’s financial information, including disbursements for clothing and property signed for by Xxxxxx Youth/NMD, and allowances received by, and signed for by, Xxxxxx Youth/NMD; CJC0821-00 Page 20 of 34 February 9, 2021 Diagnostic studies; Intake interviews conducted with Xxxxxx Youth/NMD; Special Incident Report; Written quarterly evaluations; Child and Adolescent Needs and Strengths assessment and/or other mental health assessments; Mental health treatment plan and mental health program progress notes; Records of medical/dental visits and treatment, including a copy of CHDP physical or its equivalent that is less than one (1) year old; Xxxxxx Youth/NMD’s monthly weight chart; Psychotropic medication orders/scripts and forms regarding psychotropic medication, including, but not limited to, JV-220, JV-220(A), JV-220(B), and court approved JV-223; Monthly feedback from the Xxxxxx Youth’s/NMD’s school regarding academic and social performance; and Xxxxxx Youth’s most current IEP.

  • MARC Records When applicable to the Licensed Materials, at Licensee’s request, Licensor shall provide full OCLC-quality batched sets of MARC records incorporating Licensee specifications at no additional cost by the date of the execution of this License Agreement. Updates to existing records and new title records, matching the schedule of release and delivery of new publications, will be provided on a mutually agreed-upon schedule and in a format that renders them useful to the Licensee and/or the Participating Institutions.

  • Client Records 26.2.1 CONTRACTOR shall prepare and maintain accurate and complete records of clients served and dates and type of services provided under the terms of this Contract in a form acceptable to ADMINISTRATOR.
