Term Life Insurance Policy and Group Plan Benefits. The Board of Trustees of the District shall pay the premiums on a term life insurance policy on the life of Xxxxxxx in the amount of approximately $200,000. Xxxxxxx shall be entitled to name the beneficiary for 50% of such policy, the District being the beneficiary for the remaining 50% of policy proceeds. In addition the Board of Trustees, at District expense, shall provide Xxxxxxx with the same group health, accident, and life insurance benefits and dental benefits as are provided to other administrators and the members of the Cabinet of the District, each vesting at the time and under the same circumstances as other employees. During the Term of this Agreement the District agrees to pay for, and Xxxxxxx agrees to undergo, an annual physical examination, and to advise the District of any physical, mental or other disabilities, conditions or limitations which may affect his ability to adequately and completely perform his job, or any reasonable accommodations which may be necessary, within 30 days of such examination. District shall be entitled to Xxxxxxx’ medical/mental health information in no lesser or greater amount than allowed by applicable state and/or federal law including, but not limited to, FMLA and ADA. In addition to the foregoing benefits, Xxxxxxx shall be granted the same benefits as are presently granted by the District to members of other employee units and such other and additional benefits as may be mutually agreed in writing between the Board of Trustees and Xxxxxxx.

  • Life Insurance Benefits A. During the life of this Agreement, the basic life insurance benefit made available to Faculty members shall be calculated as 3 times base annual earnings, rounded to the next highest $1,000, but not more than $225,000. A separate additional benefit up to the amount of the life insurance will be paid for accidental death and dismemberment, or loss of sight. The amount of Life and Accidental Death and Dismemberment/Loss of Sight benefits will be reduced to 65% at age 65, and further reduced (from the original insurance amount) as follows: to 50% at age 70, and 35% at age 75. Basic life insurance and AD&D benefits will be provided with no employee contributions.

  • Group Life Insurance Plan Section 1 - Eligibility Regular full-time and regular part-time employees who are on staff January 1, 1979 or who join the staff following this date shall, upon completion of the three-month probationary period, become members of the Group Life Insurance Plan as a condition of employment.

  • Group Term Life Insurance The School District will pay the full premium for each $1,000 of coverage for group term life insurance. The amount of life insurance provided will be $20,000, subject to the conditions of the carrier.

This applies to those employees with family health coverage only.

  • Hospitals of Ontario Voluntary Life Insurance Plan The Hospital also agrees to make the Hospitals of Ontario Voluntary Life Insurance Plan (HOOVLIP) available to the nurses subject to the provisions of HOOVLIP at no cost to the Hospital.

  • GROUP INSURANCE PLANS 15.01 The Company agrees to provide all full time employees with one (1) or more years of continuous service, a *Short Term Disability Benefit (S.T.D.) as generally described in Section B of a notice to all employees of the amended Benefit Plan dated May 1, 1993. *Payment for Short Term Disability shall begin on the third (3rd) consecutive day of absence. For those employees who have completed ninety (90) days of perfect attendance from the last date of return to work from an absence due to sickness or accident, shall be paid S.T.D. from the first (1st) day for the first covered absence following the ninety (90) days of perfect attendance.

  • Group Life Insurance The Hospital shall contribute one hundred percent (100%) toward the monthly premium of HOOGLIP or other equivalent group life insurance plan in effect for eligible full-time employees in the active employ of the Hospital on the eligibility conditions set out in the existing Agreements.
