Task Specific Prescription Eyewear. This letter is to confirm the conditions of obtaining task specific eyewear under the company’s safety glass program. Computer operators who wear bi-focals or tri-focals may be eligible for “ComfortEyes” (task specific eyewear) in lieu of bi-focal or tri-focal lenses. To be eligible, a prior ergonomic assessment must be completed on the employee’s job station indicating a need for “ComfortEyes”, an ergonomic program vision questionnaire must be completed, a medical specialist’s report must be obtained recommending “ComfortEyes”, and the employee must be regularly assigned and working at a computer for at least 6 hours daily at a work station.
