Service Award Payments. At least thirty (30) days before the Opt-Out and Objection Deadlines, Class Counsel will file a Fee Application that will include a request for Service Award Payments for the Settlement Class Representatives in recognition for their contributions to this Litigation. Xxxxx Health agrees not to oppose Class Counsel’s request for a service award not to exceed $3,500.00 for each class representative. The Settlement Administrator shall make the Service Award Payments to the Settlement Class Representatives from the Settlement Fund. Such Service Award Payment shall be paid by the Settlement Administrator, in the amount approved by the Court, no later than thirty (30) days after the Effective Date.
Service Award Payments. At least fourteen (14) days before the Opt-Out and Objection Deadlines, Class Counsel will file a motion seeking a service award payment for the Class Representatives in recognition for their contributions to this Action. Xxxx agrees not to oppose Class Counsel’s request for a service award not to exceed Four Thousand Dollars and Zero Cents ($4,000.00) per representative, for a total of Twelve Thousand Dollars and Zero Cents ($12,000.00). The Settlement Administrator shall make the Service Award Payments to the Class Representatives from the Settlement Fund. Such Service Award Payment shall be paid by the Settlement Administrator, in the amount approved by the Court, no later than thirty (30) days after the Effective Date.

Such Service Award Payment shall be paid by the Settlement Administrator, in the amount approved by the Court, no later than 7 days after the Effective Date.

This incentive award shall be separate and apart from any other sums agreed under this Settlement Term Sheet. Such Service Award Payment shall be paid by the Settlement Administrator, in the amount approved by the Court, no later than thirty (30) days after the Effective Date.
