SERIOUS UNTOWARD INCIDENTS. The serious untoward incident policy will be consistent with the guidance issued by the National Patients Safety Agency. The Trust’s policy will refer to Council led safeguarding arrangements for children and adults. The Trust will notify its partner immediately of critical incidents, and provide quarterly reports to the Mental Health Partnership Board. Where reports are of a confidential nature such as those that would breach rights of users to confidentiality, these will be reported outside any proceedings of the partners that are held in public.

SERIOUS UNTOWARD INCIDENTS. 1.1. The Provider must investigate serious and adverse incidents and shall provide a summary of such incidents to the Commissioner and Responsible Health Board on a monthly basis, or by exception of any incident rated as 15 or more within 24 hours of occurrence.
