Review of Performance Following Discipline. Upon request from a nurse who 22 has received discipline, the Service Center will review the nurse’s performance 23 and provide a written summary addressing the nurse’s efforts at resolving the 24 issues that led to the discipline. In responding to such requests, the time between 25 the original disciplinary action and the nurse’s request for a follow up review may 26 be taken into account and reflected in the summary. The statement will be given 27 to the nurse and placed in the nurse’s personnel file. This review is only 28 available where the nurse has received the discipline at least 18 months, but no 29 more than 36 months, prior to the time of the request.
