Prepare Delineation Report. Consultant will prepare a formal delineation report based on results from Subtask 1.1-1.3 above. The report will include all content (e.g. mapping, photos, GPS, coordinate tables, boundary rationales, field data sheets, USACE Guidance interpretation for each wetland, and/or jurisdictional status of any wetlands on the property etc.) that identify features that could potentially be subject to the USACE jurisdiction under Section 404 of the Clean Water Act and/or Section 10 of the Rivers and Harbors Act and that is required by the USACE's current procedures and for inclusion in requests for a Jurisdictional Determination. Per 33 CFR 329.12(a)(2) Shoreward limit of jurisdiction, navigable waters of the United States extend to the line on the shore reached by the plane of the Mean (average) High Water (MHW), which is the shoreward limit of Section 10 waters. Per 33 CFR 328.3(d). waters of the United States (Section 404) extend shoreward to the High Tide Line (HTL), which is the line of intersection of the land with the water’s surface at the maximum height reached by a rising tide. Maps shall be correctly demarcated with the MHW and HTL. Acreages for both sea grass beds and delineated WOUS and wetlands will be provided on each map. Consultant will provide the Authority a draft report. Consultant will incorporate Authority input into the report for a finalized version for Authority’s reference. Consultant will submit all documents in Microsoft Word and a final compiled .pdf document for Authority’s use. Consultant will provide geospatial data of all sample locations in the formats/files described in Attachment A. Task 2 - USACE Jurisdictional Determination Consultant will prepare either a Preliminary Jurisdiction Determination (PJD) form or Approved Jurisdictional Determination (AJD) form and submit all necessary supporting documents to the USACE on behalf of the Authority for review. Consultant will also provide field support to verify the results of the wetland delineation report for the Jurisdictional Determination site visit with the USACE.

  • Inspection Report The Client and the Company agree that the Company, and its inspector(s), will prepare a written home inspection report which shall:

  • Condition Report The landlord and tenant are required to inspect the residential unit together at the beginning and end of the tenancy and complete a written condition report. If the landlord allows the tenant to have a pet after the start of the tenancy, an inspection report must be done on the day the tenant starts keeping a pet or on another day mutually agreed to by the landlord and tenant, unless the tenancy started on or after January 1, 2004, and a condition inspection report was completed at that time. A report may describe any damage, how clean each room is, and the general condition of the residential unit including: the floors, carpets, appliances, and paint on the walls. The report must be signed and dated by both the landlord and the tenant who made the inspection, and each should keep a copy. Change of Landlord – A new landlord has the same rights and duties as the previous one and must follow all the terms of this agreement unless the tenant and new landlord agree to other terms.

  • Evaluation Reports Where a formal evaluation of an employee's performance is carried out, the employee shall be provided with a copy to read and review. Provision shall be made on the evaluation form for an employee to sign it. The form shall provide for the employee's signature in two (2) places, one indicating that the employee has read and accepts the evaluation, and the other indicating that the employee disagrees with the evaluation. The employee shall sign in one of the places provided within seven (7) calendar days. No employee may initiate a grievance regarding the contents of an evaluation report unless the signature indicates disagreement with the evaluation. The employee shall receive a copy of the evaluation report at the time of signing. An evaluation report shall not be changed after an employee has signed it, without the knowledge of the employee, and any such changes shall be subject to the grievance procedure.

  • Construction Reports Each Constructing Entity shall issue reports to each other Construction Party on a monthly basis, and at such other times as reasonably requested, regarding the status of the construction and installation of the Interconnection Facilities. Each Construction Party shall promptly identify, and shall notify each other Construction Party of, any event that the Construction Party reasonably expects may delay completion, or may significantly increase the cost, of the Interconnection Facilities. Should a Construction Party report such an event, Transmission Provider shall, within fifteen days of such notification, convene a technical meeting of the Construction Parties to evaluate schedule alternatives.

  • Evaluation Report 20.5.4.1 The summary evaluation report shall be prepared by the Faculty Evaluation Committee and administrative evaluator(s) and shall include each evaluator’s individual rating. The summary evaluation report shall take into account the results of each of the evaluation components (Section 20.5.3) in order to arrive at an overall rating. When the committee and the administrative evaluator(s) cannot reach an agreement as to the overall rating, the report must include written explanation.

  • Project Completion Report At the completion of construction and once a Project is placed in service, the Subrecipient must submit a Project Completion Report that includes the total number of units built and leased, affordable units built and leased, DR-MHP units built and leased, an accomplishment narrative, and the tenants names, demographics and income for each DR-MHP unit.

  • Utilization Reports Both the Contractor and Subcontractor shall complete Quarterly Utilization Reports (or similar type documents containing the same information) and submit them to the Contracting Officer and to the Bureau within ten (10) business days at the end of each quarter. This information will be used to determine the actual dollar amount paid to Subcontractor and will also serve as a record of fulfillment of Contractor’s Small Diverse Business and Small Business Commitments. If there was no activity during the quarter, then the form must be completed by stating “No activity in this quarter.” A late fee of $100.00 per day may be assessed against the Contractor if its Utilization Report is not submitted in accordance with the schedule above.
