Person-Centered. The planning process focuses on the individual, not the system or the individual’s family, guardian, or friends. The individual’s goals, interests, desires, and preferences are identified with an optimistic view of the future and plans for a satisfying life. The planning process is used whenever the individual wants or needs it, rather than viewed as an annual event.

Person-Centered. The coordinated entry process incorporates participant choice, which may be facilitated by questions in the assessment tool or through other methods. Choice can include location and type of housing, level of services, and other options about which households can participate in decisions.
Person-Centered. Person-centered care and person-centered planning in SCO means providing Covered Services, flexible benefits, and coordination to an Enrollee to meet the individual’s needs and goals. Placed at the center of care and planning, the Enrollee and their needs, preferences, and goals provide direction and purpose for what and how medical, behavioral health, Long-term Services and Supports, and flexible benefits are provided, including to address social determinants of health and health equity. This is achieved by integrating available resources of the MassHealth and Medicare programs as defined in the Contract.
Person-Centered. A term which refers to mutually beneficial partnerships between patients, their families and those delivering healthcare services, which respect individual needs and values and which demonstrate compassion, continuity, clear communication and shared decision making.
