Government Funded Benefits. Where the Beneficiary is eligible for Government-Funded Benefits, at the request of the Holder and upon completion and delivery of all forms required under the Applicable Legislation, the Agent will apply for such Government-Funded Benefits in respect of the Beneficiary. Neither the Trustee nor the Agent is responsible for determining whether the Beneficiary is eligible for Government-Funded Benefits. The Trustee shall pay out of the Assets any refund of Government-Funded Benefits required under the Applicable Legislation.
Government Funded Benefits. Desjardins Trust Inc., as Issuer of the Plan, shall ensure that an application for the Canada Disability Savings Grant (CDSG), and, if applicable, for the Canada Disability Savings Bond (CDSB), are filed as agreed. Once granted, the CDSG and, if applicable, the CDSB, will be invested as instructed by the Holder(s). The Issuer will make the necessary grant refunds pursuant to the provisions of the CDSA. The plan will comply with the conditions set out in the Canada Disability Savings Act and the Canada Disability Savings Regulations. The Holder(s) agree(s) to provide the information requested by Desjardins Trust Inc. as Issuer to enable it to apply and administer the CDSG and the CDSB.

  • Covered Benefits and Services The Contractor shall provide to its Hoosier Healthwise members, at a minimum, all benefits and services deemed “medically reasonable and necessary” and covered by the IHCP, and included in the Indiana Administrative Code and under the Contract with the State. A covered service is considered medically necessary if it meets the definition as set forth in 405 IAC 5-2-17. The Contractor shall deliver covered services sufficient in amount, duration or scope to reasonably expect that provision of such services would achieve the purpose of the furnished services.
