FINANCIAL INCENTIVES AS PART OF MEDICAL, HOSPITAL, AND PRESCRIPTION PLANS. A. If an employee elects to drop health benefits for all dependents, the annual incentive is $750; to drop all health benefits, $1,500. Cash incentives are not a part of annual salary but are added each year after the incentive amount has been determined. Employees who drop either individual, dependent or full family health benefits coverage will be allowed to re-elect district provided coverage only under the condition that a "change of status" occurred as defined below (Proof of other acceptable coverage is required.):

  • Physician Incentive Plans In the event Provider participates in a physician incentive plan (“PIP”) under the Agreement, Provider agrees that such PIPs must comply with 42 CFR 417.479, 42 CFR 438.3, 42 CFR 422.208, and 42 CFR 422.210, as may be amended from time to time. Neither United nor Provider may make a specific payment directly or indirectly under a PIP to a physician or physician group as an inducement to reduce or limit Medically Necessary services furnished to an individual Covered Person. PIPs must not contain provisions that provide incentives, monetary or otherwise, for the withholding of services that meet the definition of Medical Necessity.

This agreement describes the benefits, exclusions, conditions and limitations provided under your plan. It shall be construed under and shall be governed by the applicable laws and regulations of the State of Rhode Island and federal law as amended from time to time. It replaces any agreement previously issued to you. If this agreement changes, an amendment or new agreement will be provided.
