Incentive Program. Members who are rated as either Level I, Level II or Level III in every phase of the Physical Fitness Test are eligible to participate in the Incentive Program.
Incentive Program. The Employer, at its sole discretion, may from time to time implement incentive programs for individuals or groups consisting of awards and/or cash and/or refreshments (For example: coffee and donuts) in recognition of performance improvements, innovative ideas resulting in savings and/or benefits, or other similar improvements that are work related and can be documented and measured. The Employer agrees to furnish the Union with a written copy of the Employer’s incentive plans as they are developed and/or amended from time to time. The Union may withdraw from participation in the program at any time during the life of this Agreement upon written notice to the Employer.
Incentive Program. The sick leave incentive program is intended to discourage unnecessary use of sick leave by rewarding employees who do not use sick leave in a given time period. Under the City program, all members who are not ill for one quarter as defined by Xxxxxxx (e.g., January—March) will receive four (4) additional hours of vacation. For prescheduled medical and/or dental appointments (requiring 24-hour notice) vacation, comp time, or personal leave may be utilized in order for the employee’s sick leave incentive program not to be adversely affected.
Incentive Program. At its sole discretion, the Employer may from time to time elect to establish incentive programs for individuals or groups which may consist of cash or other awards in recognition of improved job performance, improved safety records, innovative ideas that result in savings or other benefits, or other similar work related improvements, provided the Union is informed in writing of any such programs.
Incentive Program. Pursuant to Section A(2)(a) of Exhibit B in the Agency Agreement (Rev. April 2017) and the Agent Agreement (Rev. July 2016), the Exchange hereby updates its Schedule of Commissions to include a performance incentive or “bonus” program that provides additional compensation above the standard agent commissions. The program applies to new Covered California for Small Business (CCSB) groups with initial effective dates of 7/1/20, 8/1/20, 9/1/20, 10/1/20, 11/1/20, 12/1/20, 1/1/21. Agencies and Agents must strictly comply with the program rules to qualify for incentive payments. The Exchange shall not be liable for any payments after cancellation of the program. The Exchange retains sole discretion to determine compliance with the program rules. As set forth below, the Exchange will pay incentive payments to Agency in addition to standard commission payments as set forth in Sections A(2)(a) of Exhibit B in this Agreement. The Exchange shall report the incentive payments on Forms 1099 and 5500, as well as any other reporting as required by law. The Incentive Program as set forth in this section is subject to cancellation or modification at any time by the Exchange without prior notice to Agency. The Exchange will make a good faith effort to notify Agency of any modifications to the Incentive Program. The Exchange shall not be liable for payment of any incentive payments after cancellation of the program. Incentive Program. Employees who do not exceed 1 sick day per year will be paid a stipend of $250.00. Employees who do not exceed 2 sick days per year will be paid a stipend of $125.00. Employees must work a full school year to attain the incentive.

Incentive Program. During the Term, the Employee shall be eligible for incentive compensation in accordance with a written incentive program mutually established by the Chairman and the Employee on an annual basis (the "Incentive Program"). The Incentive Program shall provide that sixty-five percent (65%) of the maximum incentive that may be earned by the Employee shall be based on compliance goals mutually established by the Chairman and the Employee and thirty-five percent (35%) of the maximum incentive that may be earned by the Employee shall be based on the financial performance of the Company and its subsidiaries as a whole. All incentive payments under the Incentive Program shall be determined quarterly, and shall be calculated by reference to the incentive percentage earned by the Employee multiplied by the Base Salary actually paid to the Employee for the calendar quarter for which the incentive is determined. The maximum percentage of the Employee's Base Salary that the Employee may earn under the Incentive Program shall be forty percent (40%) of the Base Salary actually paid to the Employee for the calendar quarter for which the incentive is determined. Any quarterly incentive earned by the Employee for any calendar quarter shall be paid by the Company to the Employee not later than sixty (60) days after the end of such calendar quarter. Any amounts earned by the Employee under the Incentive Program shall be hereinafter referred to as the "Performance Incentive."
Incentive Program. Health Plans will be eligible to participate in the CHCUP incentive program when the Health Plan has exceeded both the sixty percent (60%) state screening rate and the federal eighty percent (80%) participation and screening ratio goals as outlined in Attachment II, Section V, Covered Services. The Agency will determine which health plans will participate based upon the audited CHCUP reports submitted each October as set forth in Attachment II, Section XII, Reporting Requirements.
Incentive Program. AVEO shall provide, at AVEO’s cost, the AVEO Sales Force with an incentive or bonus plan in connection with the promotion of the Licensed Product. Prior to implementation, AVEO shall provide a copy of such incentive or bonus plan to Schering-Plough for its review and approval, such approval not be unreasonably withheld, conditioned or delayed.
