Data Quality Assurance Protocol. An annual review and verification/revision of a formal Data Quality Assurance Protocol that includes a plan for communication, training, and monitoring/oversight of the collection, maintenance, submission and reporting of data. Protocol needs to designate a position responsible for ensuring the quality of data. A separate protocol does not need to be submitted if it is included in the Provider’s CQI. 30 days from (a) contract effective date and (b) when revised.
