Chronic Disease Management Program. The County shall develop and implement a written chronic care disease management program, consistent with generally accepted correctional standards, which provides Detainees suffering from chronic illnesses with appropriate diagnosis, treatment, monitoring, and continuity of care. As part of this program, The County shall maintain a registry of Detainees with chronic diseases. Without admitting prior deficiencies, the County of Oklahoma County will continue striving to provide constitutional standards of care to all detainees and inmates at the Jail.
