CHANGE OF BENEFICIARY. 18.1 The policyholder has the authority to appoint another beneficiary during the life of the insured person.. However, if the beneficiary has declared, with the written consent of the policyholder, that he accepts the benefit of the contract, the policyholder can exercise his rights under the contract only with the cooperation of the beneficiary, who has so accepted. The change will take effect from the moment that the insurer has noted this on the policy.

CHANGE OF BENEFICIARY. The Owner may change any Beneficiary unless otherwise provided in the previous designation by providing a Notice to change beneficiary. A change of Beneficiary will revoke any previous designation. When a change of Beneficiary is received by LNY, whether or not the Owner is then alive, it will take effect as of the date the request was sent. For purposes of determining on which date a written change of Beneficiary is sent, the postmark date will be used. Any payment made or action taken or allowed before the change of Beneficiary is received will be without prejudice to LNY. LNY reserves the right to request the Contract for endorsement of the change.
CHANGE OF BENEFICIARY. The Owner may change the Beneficiary by written Request. The Company shall not be bound by any change of Beneficiary unless it is made in writing and recorded at the Retirement Resource Operations Center. A change of Beneficiary will take effect as of the date the written Request was signed, subject to receipt at the Retirement Resource Operations Center, unless a certain date is specified by the Owner. If an Owner dies before the date the Request was processed, the change will take effect as of the date of the Request, unless the Company has already made a payout or has otherwise taken action on a designation or change before receipt or processing of such Request. If an Owner dies, and the surviving Joint Owner is the surviving Spouse of the deceased Owner, the surviving Spouse will become the Beneficiary and may take the death benefit or elect to continue this Contract in force. An irrevocable Beneficiary designation may not be changed without the written consent of that Beneficiary, except to the extent required by law.

CHANGE OF BENEFICIARY. The right to change the beneficiary is reserved to the employee, provided there is no legal restriction to the contrary, but such change shall take effect only upon receipt of a written request by the Employer. Whenever a change of beneficiary is desired a completed Change of Beneficiary Form should be submitted to the Personnel & Industrial Relations Department in order that the change may be recorded. If the beneficiary named should predecease the employee another beneficiary should be named immediately. Any addition to the initial amount of insurance will be paid to the person to whom such initial amount is payable unless another beneficiary for the additional amount shall have been validly appointed.
CHANGE OF BENEFICIARY. Subject to the rights of any irrevocable Beneficiary(ies), the Contract Owner may change the Primary Beneficiary(ies) or Contingent Beneficiary(ies). Any change must be made by Written Request. The change will take effect as of the date the notice is signed. The Company will not be liable for any payment made or action taken before it records the change.

CHANGE OF BENEFICIARY. The Certificate Holder may change the Beneficiary. A written request, dated and signed by the Certificate Holder, must be filed at our Home Office. If there are joint Certificate Holders, both must sign the request. After the change is recorded, it will take effect as of the date the request was signed. If the request reaches our Home Office and is recorded after the Certificate Holder dies, but before any payment is made, the change is valid.
CHANGE OF BENEFICIARY. You may, upon Authorized Request, change the Beneficiary(ies) subject to the following limitations. For solely owned individual or non-individual contracts, you may change the Beneficiary(ies) at any time before an Owner's death, if individually owned, or before an Annuitant's death, if non-individually owned. For jointly owned contracts, you may change the contingent Beneficiary(ies) at any time. Upon the first Joint Owner's death, the surviving Joint Owner is the sole primary Beneficiary and may not be changed. An irrevocable Beneficiary must give written consent before we will change the Beneficiary. A Beneficiary change takes effect as of the date you signed the Authorized Request. We are not liable for any actions we take or payments we make before the Service Center receives the Authorized Request.
CHANGE OF BENEFICIARY. You may change the beneficiary anytime while the annuitant is living by satisfactory written request to us. Once the change is recorded by us, it will take effect as of the date of your request, subject to any action taken or payment made by us before the recording.
