Application Of Catalog. This Catalog contains the terms and conditions and price lists applicable to Private Line Transport Services, furnished by Qwest Corporation, d/b/a CenturyLink QC hereinafter referred to as the Company, between and among points within the State of North Dakota. The regulated services offered herein by Qwest Corporation, d/b/a CenturyLink QC, whether under that name, or the trade or brand name CenturyLink, are subject to the terms and conditions of this Catalog. The procedures which will be followed by the Company in rendering service are set forth in Section 2, General Regulations, and Section 3, Rate Regulations, following, and 2.6, which includes definitions explaining phrases and terms used. Unless otherwise specified, the terms and conditions and price lists are applicable to intraLATA/Intrastate Private Line Transport Services furnished by the Company or by the Company in conjunction with another connecting Company. The provision of such services by the Company, as set forth in this Catalog, does not constitute a joint undertaking with the customer for the furnishing of any service. Prices for Private Line Transport Service will be above cost. The Company will make every reasonable effort to provide the services delineated in Section 5, following, upon reasonable notice of request for the service from the customer. All rates and charges shown in this Catalog provide for the furnishing of service where suitable facilities are available. Where special construction of channel facilities is necessary, special construction charges may apply. Terms and conditions governing the determination of charges for special construction in connection with Private Line Transport Services furnished by the Company are found in 4.1.6, Special Construction. Each page of the Catalog bears the date upon which it became effective. The Table of Contents contains a complete list of all effective Private Line Transport Services offerings and includes the titles, page numbers and other information necessary for their identification.
