Sample Receipt Forms definition

Sample Receipt Forms means those multi-part forms, whether paper or electronic, supplied by Impax or Impax’s Permitted Subcontractor for the purpose of recording Detail and Sample activity performed by Impax PSRs during Sales Calls. These forms shall also be used as Sample receipts on which to obtain a Neurologist’s signature in acknowledgment of receipt of Samples. Each Sample Receipt Form shall include, at a minimum, the information set forth in Schedule 1.79.
Sample Receipt Forms means those multi-part paper forms (or an electronic version thereof) supplied by AHPC for the purpose of recording Detail and Sample activity performed by ViroPharma sales representatives during Details. These forms are also used as Sample receipts on which to obtain a physician's signature in acknowledgment of the physician's receipt of Samples.
Sample Receipt Forms means those multi-part paper or electronic forms used for the purpose of recording Detail and Sample request and receipt activity performed by PSRs during Sales Calls. These forms are also used as Sample receipts on which to obtain a physician's signature in acknowledgment of the physician's receipt of a Sample. The Sample Receipt Forms will be in a form agreed to by the JCC.

Examples of Sample Receipt Forms in a sentence

  • Impax shall ensure that each Impax PSR fills out the Sample Receipt Forms accurately, completely and timely.

  • A copy of all such Sample Receipt Forms shall be kept by Impax or its Permitted Subcontractor.

  • Such professional sales representative shall mail to the Distributing Party, in pre-addressed, postage-paid envelopes provided by the Distributing Party, at the close of each business day the original completed Sample Receipt Forms for all Details performed by such professional sales representative that day.

  • Impax also shall keep its copies of the completed Sample Receipt Forms.
