Root Cause Analysis definition

Root Cause Analysis means a method of problem solving designed to identify the underlying causes of a problem. The focus of a root cause analysis is on systems, processes, and outcomes that require change to reduce the risk of harm.
Root Cause Analysis is the formal process, specified in the Policy and Procedures Manual, to be used by Supplier to diagnose problems at the lowest reasonable level so that corrective action can be taken that will eliminate, to the extent reasonably possible, repeat failures.
Root Cause Analysis means a Problem analysis process undertaken to identify and quantify the underlying cause(s) of a Problem, and document the necessary corrective actions to be taken to prevent recurring Problem, or as otherwise defined in Annexure I from time to time.
