Root Cause Analysis definition

Root Cause Analysis means a method of problem solving designed to identify the underlying causes of a problem. The focus of a root cause analysis is on systems, processes, and outcomes that require change to reduce the risk of harm.
Root Cause Analysis is the formal process, specified in the Policy and Procedures Manual, to be used by Supplier to diagnose problems at the lowest reasonable level so that corrective action can be taken that will eliminate, to the extent reasonably possible, repeat failures.
Root Cause Analysis is a Problem Management analysis process used in determining and documenting the unknown underlying cause(s) (e.g., root causes) of one or more Incidents and ensuring that appropriate actions are taken to correct the situation to minimize the possibility of recurrence of the Incident(s) or events.
