Home Health Care Agency definition

Home Health Care Agency means an agency or organization which provides a program of home health care and which:
Home Health Care Agency means an organization that provides skilled nursing services and therapeutic services (home health aide services, physical therapy, occupational therapy, speech therapy, medical social services) on a visiting basis, in a place of residence used as the Covered Person’s home. The organization must be Medicare certified and licensed within the state in which home health care services are provided.

  • The Plan provides Benefits for home health care services when services are performed and billed by a Home Health Care Agency.

  • These visits may be provided either in the Physician’s office or in the Member’s home by a Home Health Care Agency.

  • Any Hospital, ambulatory care facility, Chemical Dependency Treatment Facility, Skilled Nursing facility, Home Health Care Agency or mental health facility, as defined in this Certificate.

  • Any Physician, health care practitioner, pharmacy, supplier or facility, including, but not limited to, a Hospital, clinical laboratory, Ambulatory Surgery Center, Retail Health Clinic, Skilled Nursing Facility, Long Term Acute Care facility, or Home Health Care Agency holding all licenses required by law to provide health care services.

  • A Hospital, Physician, Skilled Nursing Facility, Hospice, Home Health Care Agency, other medical practitioner or provider of medical services and supplies, that does not have an In- Network Provider contract with Alliant.

Home Health Care Agency means an entity licensed by state or local law operated primarily to provide skilled nursing care and therapeutic services in an individual’s home and:
