Home Health Care Agency means an agency or organization which provides a program of home health care and which:

The organization must be Medicare certified and licensed within the state in which home health care services are provided.

  • This limit will not apply to speech, physical or occupational therapy services provided by a Home Health Care Agency pursuant to a home health plan of treatment Prior Authorized by Us. Such services will be subject to the limit, if any for Home Health Services.
