The Plan provides Benefits for home health care services when services are performed and billed by a Home Health Care Agency.
These visits may be provided either in the Physician’s office or in the Member’s home by a Home Health Care Agency.
Any Hospital, ambulatory care facility, Chemical Dependency Treatment Facility, Skilled Nursing facility, Home Health Care Agency or mental health facility, as defined in this Certificate.
