• The invoice must be an original invoice, submitted to: Chemonics Iraq LLC ICRI-Ta’afi Office Jouhayna Business Center Attention: Finance Department To constitute a proper invoice, the Subcontractor’s invoice must include the following information and/or attached documentation.

• In accordance with FAR Clause 52.243-4 Changes (JUN 2007) for fixed price construction subcontracts over the simplified acquisition threshold, or FAR clause 52.243-5 “Changes and Changed Conditions” (APR 1984) for fixed price construction subcontracts under the simplified acquisition threshold, Chemonics may at any time, by written order, and without notice to the sureties, if any, make changes within the general scope of this subcontract in the services to be performed.

• The Subcontractor shall present an invoice to Chemonics only for services and/or products that have been accepted by Chemonics.

• Before starting work, the Subcontractor must provide Chemonics with a copy of the DBA coverage policy that covers each of its employees.
