Annual Enrollment Forms definition

Annual Enrollment Forms means, for any Plan Year, the Annual Election Form, the Distribution Election Form and any other forms or documents which may be required of a Participant by the Committee, in its sole discretion.
Annual Enrollment Forms means, for any Deferral Year, the Annual Election Form, the Distribution Election Form and any other forms or documents which may be required of a Director by BSC, in its sole discretion.
Annual Enrollment Forms means, with respect to the portion of any Account that relates to a Participant’s Annual Elective Deferrals under the Plan, the Annual Election Form and the Distribution Election Form (or the Amended Distribution Election Form last signed and submitted by the Participant) with respect to that Account.

Examples of Annual Enrollment Forms in a sentence

  • The Annual Enrollment Forms submitted by a Participant in respect of a particular Plan Year will not be effective with respect to any subsequent Plan Year.

  • An Employer shall have no obligation to a Participant under the Plan except as expressly provided in the Plan and his or her Annual Enrollment Forms.

  • If the required Annual Enrollment Forms are not timely delivered for the subsequent Deferral Year, the Director shall not be eligible to make any deferrals with respect to such subsequent Deferral Year.

  • A Director may elect to commence Compensation Deferral by completing, executing and filing the Annual Enrollment Forms with BSC pursuant to Section 2.2.

  • The Annual Enrollment Forms submitted by a Director in respect of a particular Deferral Year will not be effective with respect to any subsequent Deferral Year.

  • Except as provided below, the Annual Enrollment Forms must be signed, executed and filed with BSC prior to the first day of the Deferral Year to which they apply.

  • As a condition to being eligible to make an Annual Participant Deferral for any Plan Year, each selected Eligible Deferral Employee shall complete and return to the Committee each of the Annual Enrollment Forms no later than December 31st of the immediately preceding Plan Year, or such earlier date as the Committee may establish from time to time and in accordance with the requirements of Section 409A.

  • Provided an Eligible Deferral Employee has met all enrollment requirements set forth in the Plan in respect of a particular Plan Year and any other requirements imposed by the Committee, including signing and submitting all Annual Enrollment Forms to the Committee within the specified time period, the Eligible Deferral Employee’s designated deferrals shall commence as of the first payment date of the particular Plan Year.

  • An Employer’s liability for the payment of benefits shall be defined only by the Plan and the Annual Enrollment Forms, as entered into between an Employer and a Participant.

  • In the case of a Newly Eligible Director, Compensation Deferral shall commence as of the date such Director’s Annual Enrollment Forms are received by BSC, but no later than 30 days following the date on which such Director first became eligible to participate in the Plan, and such Annual Election Form shall apply only with respect to Compensation earned for services performed subsequent to the time such Annual Election Form is received by BSC.

  • Enrollment Form means an agreement pursuant to which an Eligible Employee may elect to enroll in the Plan, to authorize a new level of payroll deductions, or to stop payroll deductions and withdraw from an Offering Period.

  • Annual Enrollment Period or "Enrollment Period" means the period before each policy anniversary so designated by the Sponsor and Lincoln during which you may enroll for coverage under this policy.

  • Enrollment means the number of students who are enrolled in a school operated by the district on October 1. A student shall be counted as one whether the student is enrolled as a full-time or part-time student. Students enrolled in prekindergarten programs shall not be counted.

  • Enrollment Agreement means an agreement between the Company and an employee, in such form as may be established by the Company from time to time, pursuant to which the employee elects to participate in this Plan, or elects changes with respect to such participation as permitted under the Plan.

  • Special Enrollment Period means a period of time allowed under this Plan, other than the eligible person’s Initial Enrollment Period or an Open Enrollment Period, during which an eligible person can request coverage under this Plan as a result of certain events that create special enrollment rights.

  • Open Enrollment Period means an enrollment period during which time any Eligible Employee and/or Dependent may apply to become a Subscriber and/or Covered Dependent, and existing Subscribers may apply to change to another provider network or coverage option, if available, or elect to terminate coverage.

  • Deferral Election Form means the form established from time to time by the Plan Administrator that the Director completes, signs and returns to the Plan Administrator to designate the amount of the Deferrals.

  • Election Form means the form established from time to time by the Committee that a Participant completes, signs and returns to the Committee to make an election under the Plan.

  • Enrollment Date means the first day of each Offering Period.

  • Enrollment Period means the period agreed upon by the Policyholder and Us when an Eligible Person may enroll for coverage or an Insured may change benefit elections under the Policy.

  • Deferral Year means each calendar year during which the Director makes, or is entitled to make, Compensation Deferrals under Section 3 hereof.

  • Deferral Election means the Participant’s election under Section 3.1 to defer all or a portion of his or her Compensation.

  • Distribution Election Form means the form established from time to time by the Plan Administrator that the Director completes, signs and returns to the Plan Administrator to designate the time and form of distribution.

  • Disenrollment means either voluntary or involuntary termination of a participant from the Independent Choices Program.

  • Effective date of enrollment means the first date when an en- rollee is entitled to receive covered benefits.

  • Distribution Election With respect to any Series, as specified in the related Supplement.

  • Deferral Agreement means an irrevocable agreement entered into between a Nonemployee Director and the Company to authorize the Company to reduce the amount of the Nonemployee Director’s Annual Retainer and credit the amount of such reduction to the Plan consistent with the requirements of Section 409A of the Code.
