Reporting Non-Permitted Use or Disclosure. Business Associate shall report to Covered Entity in writing each Use or Disclosure that is made by Business Associate, its employees, representatives, agents or subcontractors that is not specifically permitted by this BAA, of which Business Associate becomes aware, as well as each Security Incident, as defined in the Security Rule, of which Business Associate becomes aware and any Breaches of PHI as defined by federal and state laws of which Business Associate becomes aware. An initial report shall be made within five (5) business days from the time the Business Associate becomes aware of the non-permitted Use or Disclosure or Security Incident, followed by a written report, including risk assessment, to Covered Entity no later than fifteen (15) business days from the date the Business Associate becomes aware of the non-permitted Use or Disclosure or Security Incident.
Reporting Non-Permitted Use or Disclosure. MBR shall not Use or Disclose Health Information except as permitted by this Agreement or as required by law. MBR shall report to Client a Use or Disclosure that is made by MBR that is not permitted by this Agreement or which MBR becomes aware.
Reporting Non-Permitted Use or Disclosure. Business Associate will promptly report to the Plan any use or disclosure of Individuals’ PHI not permitted by this Agreement or in writing by the Plan or its Plan administrator, of which Business Associate becomes aware. In addition to reporting to Plan any use or disclosure of PHI not permitted by the Agreement, Business Associate will also report any security incidents of which Business Associate becomes aware. A security incident is an attempted or successful unauthorized access, use, disclosure, modification or destruction of information or interference with system operations in an information system.

Morehouse shall report to the Participant each Use or Disclosure that is made by Morehouse, its employees, representatives, agents or subcontractors that is not specifically permitted by this Agreement. The initial report shall be made by telephone call to the Participant within forty-eight (48) hours from the time the Morehouse becomes aware of the non-permitted Use or Disclosure, followed by a written report to Participant no later than ten (10) business days from the date the Morehouse becomes aware of the non-permitted Use or Disclosure.
Reporting Non-Permitted Use or Disclosure. Business Associate shall report to Friday Colorado any Use or Disclosure of PHI in its possession of which it becomes aware that is not permitted by the terms of this Agreement, contractual arrangement or as may be required by law. Additionally, Business Associate shall report to Company any attempted or successful (a) unauthorized access, use, disclosure, modification, or destruction of Company’s Electronic PHI; or (b) interference with Business Associate’s system operations in Business Associate’s information systems, of which Business Associate becomes aware.

FAI will report to Plan any use or disclosure of Members’ Protected Health Information not permitted by this Business Associate Agreement or in writing by Plan. FAI will make the report to Plan not more than ten (10) business days after FAI learns of such non-permitted use or disclosure.

Reporting Non-Permitted Use or Disclosure. CHI shall immediately notify Hospital of each Use or Disclosure, of which it becomes aware, that is made by CHI, its employees, representatives, agents or subcontractors that is not specifically permitted by this Agreement.
